摘要
目的探讨诊断甲状腺微小癌时应用超声引导下甲状腺细针穿刺的效果。方法116例疑似甲状腺微小癌患者,均在超声引导下行甲状腺细针穿刺进行诊断,统计患者在超声引导下甲状腺细针穿刺的检查结果,并与术后病理结果进行对比分析。结果超声引导下甲状腺细针穿刺检查结果:116例患者中良性病变73例;甲状腺癌43例,其中肿瘤直径>10 mm 25例,直径≤10 mm 18例。术后病理结果:116例患者中良性病变72例;甲状腺癌44例。肿瘤直径>10 mm 25例,包括髓样癌14例,滤泡性癌11例;直径≤10 mm 19例,均是乳头状癌,其中女14例、男5例,年龄20~59岁,平均年龄(38.69±4.27)岁。共有1例甲状腺微小癌漏诊,诊断甲状腺微小癌时应用超声引导下行甲状腺细针穿刺的检查准确率为94.74%(18/19)。结论在超声引导下行甲状腺细针穿刺诊断甲状腺微小癌的准确率较高,并且检查结果与术后病理结果非常接近,说明该方式是术前获取患者病理诊断的重要手段,临床推广价值较高。
Objective To discuss the effect of ultrasound-guided fine needle aspiration in the diagnosis of thyroid microcarcinoma.Methods There were 116 patients with suspected thyroid microcarcinoma,and all patients were diagnosed by ultrasound-guided thyroid fine needle aspiration.The examination results of ultrasound-guided thyroid fine needle aspiration were counted and analyzed in comparison with postoperative pathological findings.Results Ultrasound-guided fine needle aspiration of the thyroid gland:73 cases had benign lesions among 116 patients;43 cases had thyroid cancer,of which 25 cases with tumors>10 mm in diameter and 18 cases with tumors≤10 mm in diameter.Postoperative pathological findings:72 cases had benign lesions among 116 patients;44 cases had thyroid cancer.25 cases with tumors>10 mm in diameter,including 14 cases of medullary carcinoma and 11 cases of follicular carcinoma;19 cases with tumors≤10 mm in diameter,all of which were papillary carcinoma,including 14 female cases and 5 male cases,aged 20-59 years,with an average age of(38.69±4.27)years.1 case of thyroid microcarcinoma was missed,and the accuracy rate of ultrasoundguided fine needle aspiration of the thyroid gland was 94.74%(18/19).Conclusion Ultrasound-guided thyroid fine-needle aspiration has a high accuracy in the diagnosis of thyroid microcarcinoma,and the examination results are very close to the postoperative pathological findings,indicating that this method is an important method for obtaining patient pathological diagnosis before surgery,and it has high clinical promotion value.
