摘要
目的分析糖皮质激素联合环磷酰胺治疗系统性红斑狼疮的效果。方法选择2017年3月至2019年3月我院收治的60例系统性红斑狼疮患者作为研究对象,根据治疗方法不同将其分为对照组(28例)和治疗组(32例)。对照组给予糖皮质激素治疗,治疗组在对照组基础上给予环磷酰胺治疗。比较两组的临床疗效、不良反应发生情况、炎性因子水平、免疫功能指标、肾功能指标。结果治疗组的治疗总有效率高于对照组,差异具有统计学意义(P<0.05)。两组的不良反应总发生率无显著差异(P>0.05)。治疗后,两组的TNF-α、IL-6、IL-1β水平均降低,且治疗组低于对照组,差异具有统计学意义(P<0.05)。治疗后,两组的CD3^(+)、CD4^(+)均升高,CD8^(+)均降低,且治疗组优于对照组,差异具有统计学意义(P<0.05)。治疗后,两组的24 h尿蛋白定量、SCr、BUN水平均降低,且治疗组低于对照组,差异具有统计学意义(P<0.05)。结论糖皮质激素联合环磷酰胺治疗系统性红斑狼疮的效果显著,可有效控制病情,减轻炎症反应,改善免疫功能和肾功能,建议临床推广。
Objective To analyze the effect of glucocorticoid combined with cyclophosphamide in the treatment of systemic lupus erythematosus.Methods A total of 60 patients with systemic lupus erythematosus treated in our hospital from March 2017 to March 2019 were selected as the research objects.According to different treatment methods,the patients were divided into control group(28 cases)and treatment group(32 cases).The control group was given glucocorticoid treatment,and the treatment group was given cyclophosphamide treatment on the basis of the control group.The clinical efficacy,adverse reactions,inflammatory factors level,immune function indexes and renal function indexes were compared between the two groups.Results The total effective rate of treatment in the treatment group was higher than that in the control group,and the difference was statistically significant(P<0.05).There was no significant difference in the total incidence of adverse reactions between the two groups(P>0.05).After treatment,TNF-α,IL-6,IL-1βlevels in the two groups decreased,and those in the treatment group were lower than the control group,the differences were statistically significant(P<0.05).After treatment,CD3^(+) and CD4^(+) increased and CD8^(+) decreased in the two groups,and those in the treatment group were better than the control group,the differences were statistically significant(P<0.05).After treatment,the 24 h urinary protein quantification,SCr and BUN levels in the two groups decreased,and those in the treatment group were lower than that the control group,the differences were statistically significant(P<0.05).Conclusion Glucocorticoid combined with cyclophosphamide in the treatment of systemic lupus erythematosus has a significant effect,it can effectively control the disease,reduce inflammatory reaction,improve immune function and renal function,which is recommended to be popularized in clinic.
