摘要
目的观察龈下刮治与根面平整术(SRP)治疗慢性牙周炎(CP)伴咬合创伤患牙患者的效果。方法选取2017年6月-2020年6月我院口腔科收治的86例CP伴咬合创伤患牙作为研究对象,采用随机数字表法分为对照组与观察组,各43例。对照组先行咬合调整后进行SRP治疗,观察组则先行SRP治疗后进行咬合调整,比较两组治疗前后牙周指标[临床牙周附着丧失(CAL)、菌斑指数(PLI)、牙周探诊深度(PD)]、牙龈指标[牙龈指数(GI)、龈沟出血指数(SBI)]、咬合受力与咬合时间、龈沟液炎性介质[白细胞介素-6(IL-6)、肿瘤坏死因子-α(TNF-α)]、牙龈肿胀及探针出血情况。结果治疗后,观察组PLI、PD、CAL、GI、SBI、IL-6、TNF-α低于对照组,差异有统计学意义(P<0.05);观察组咬合受力、咬合时间均少于对照组,差异有统计学意义(P<0.05);观察组牙龈肿胀率与探针出血率均低于对照组,差异有统计学意义(P<0.05)。结论SRP有利于CP伴咬合创伤患者病情的转归,将其应用于咬合调整前,可促进牙周及牙龈指标的进一步改善,同时提高咬合矫正效果,控制炎性反应,改善牙龈肿胀及出血情况。
Objective To observe the effect of scaling and root planing(SRP)in the treatment of chronic periodontitis(CP)with occlusal trauma.Methods A total of 86 CP patients with occlusal trauma admitted to the Department of Stomatology of our hospital from June 2017 to June 2020 were selected as the study subjects.They were divided into control group and observation group by random number table method,with 43 cases in each group.The control group was treated with SRP after occlusal adjustment,while the observation group was treated with occlusal adjustment after SRP.The periodontal indexes[clinical attachment loss(CAL),plaque index(PLI),probing depth(PD)],gingival indexes[gingival index(GI),sulcus bleeding index(SBI)],occlusal force and occlusal time,sulcus fluid inflammatory mediators[interleukin-6(IL-6),tumor necrosis factor-α(TNF-α)],gingival swelling and probe bleeding were compared between the two groups before and after treatment.Results After treatment,PLI,PD,CAL,GI,SBI,IL-6 and TNF-αin the observation group were lower than those in the control group,and the differences were statistically significant(P<0.05).The occlusion force and occlusion time in the observation group were less than those in the control group,and the difference was statistically significant(P<0.05).The gingival swelling rate and probe bleeding rate in the observation group were lower than those in the control group,and the difference was statistically significant(P<0.05).Conclusion SRP is beneficial to the outcome of patients with CP with occlusal trauma.Applying it before occlusal adjustment can promote the further improvement of periodontal and gingival indicators,and at the same time improve the effect of occlusal correction,control inflammatory reactions,and improve gingival swelling and bleeding.
