摘要
目的分析口腔锥形束CT在下颌第二磨牙C形根管诊治中的应用效果。方法样本收录为该院2019年6月—2021年6月下颌第二磨牙根管治疗的患者,抽出116例样本进行分析,对所有资料采取锥形束CT(CBCT)扫描,包括横断面、冠状面及矢状面等多层面扫描,通过计算机技术对三维数据图像进行总结,为医师提供下颌第二磨牙的根管数量、牙根形态、C形根管形态等指标。结果通过CBCT在根管治疗中辅助定位C形根管具有较高的准确率;148颗牙齿中出现了55颗C形根管,占比为37.2%,包括7颗C3型变异根管,占比12.7%,19颗C2型变异根管,占比34.5%,29颗C1型变异根管,占比52.7%,对患牙采取根管再治疗后成功的有54颗,占比98.2%,术后3月对患者复查无并发症情况。结论对于根管治疗中的变异根管、疑难根管可采取CBCT进行诊断评估,能充分显示第二磨牙下根管的实际情况,为患者的治疗方案提供了基础分析内容,从而保证了患者的临床治疗效果,临床应用具有较高的直观性及准确度,在临床上具有重要的研究价值。
Objective To analyze the effect of oral cone-beam CT in the diagnosis and treatment of C-shaped root canals of mandibular second molars.Methods The samples were included in our hospital for patients undergoing root canal treatment of the mandibular second molars.116 samples were selected for analysis.The collection time was between June 2019 and June 2021.Cone beam CT(CBCT)scans were performed on all the data,including cross-sectional,coronal and sagittal planes and other multi-level scanning,through computer technology to summarize the three-dimensional data images,to provide physicians with indicators such as the number of root canals,root morphology,and C-shaped root canal morphology of the mandibular second molar.Results CBCT assisted positioning of C-shaped root canals in root canal treatment had a high accuracy rate;55 C-shaped root canals appeared in 148 teeth,accounting for 37.2%,including 7 C3 variant root canals,accounting for 12.7%,19 C2 variant root canals,accounting for 34.5%,29 C1 variant root canals,accounting for 52.7%.The success teeth after root canal retreatment was 54,accounting for 98.2%,3 months after the operation,the patient was re-examined without complications.Conclusion CBCT can be used for diagnosis and evaluation of variable root canals and difficult root canals in root canal treatment,which can fully show the actual situation of the root canals under the second molars,provide basic analysis content for the patient's treatment plan,and ensure the patient's clinical treatment effect.The clinical application has high intuitiveness and accuracy,and has important clinical research value.
