摘要
目的:分析超声联合神经刺激仪引导腰骶丛神经阻滞在老年髋部手术患者中的应用效果。方法:将2019年1-12月笔者所在医院收治的50例老年髋部手术患者作为研究对象,按照随机数字表法将其分为对照组和观察组,各25例。对照组采用单纯腰骶丛神经阻滞,观察组采用超声联合神经刺激仪引导腰骶丛神经阻滞。比较两组入室时和麻醉阻滞后的收缩压(SBP)、心率(HR)水平。比较两组镇痛效果及持续时间、术中阻滞不全发生情况。结果:入室时,两组SBP、HR水平比较差异均无统计学意义(P>0.05);麻醉阻滞后,两组SBP、HR水平均较入室时降低,但观察组降低幅度小于对照组,差异有统计学意义(P<0.05)。观察组术后2、4、8、24 h的VAS评分均低于对照组,镇痛持续时间长于对照组,差异均有统计学意义(P<0.05)。观察组术中阻滞不全发生率低于对照组,差异有统计学意义(P<0.05)。结论:超声联合神经刺激仪引导腰骶丛神经阻滞在老年髋部手术患者中的应用效果显著,能维持患者术中生命体征平稳,减轻术后疼痛度,减少术中阻滞不全的发生,值得采用。
Objective:To analyze the application effect of ultrasonography combined with nerve stimulator guided lumbosacral plexus nerve block in elderly patients undergoing hip surgery.Method:A total of 50 elderly patients undergoing hip surgery in our hospital from January to December 2019 were selected as the study subjects,and were divided into the control group and the observation group according to the random number table method,with 25 cases in each group.The control group received lumbosacral plexus nerve block alone,and the observation group received ultrasonography combined with nerve stimulator guided lumbosacral plexus nerve block.The systolic blood pressure(SBP),heart rate(HR)at entered the room and after anesthesia block were compared between the two groups.The anesthesia effect,duration and occurrence of intraoperative block insufficiency were compared between the two groups.Result:There were no significant differences in SBP and HR between the two groups at entered the room(P>0.05).After anesthesia block,the levels of SBP and HR in the two groups were decrease compared with those at entered the room,but the decrease range in the observation group was less than that in the control group,and the differences were statistically significant(P<0.05).The VAS scores of the observation group were lower than those of the control group at 2,4,8 and 24 h after surgery,and the anesthesia duration was longer than that of the control group,and the differences were statistically significant(P<0.05).The incidence of intraoperative block insufficiency in the observation group was lower than that in the control group,and the difference was statistically significant(P<0.05).Conclusion:Ultrasonography combined with nerve stimulator guided lumbosacral plexus nerve block has a significant application effect in elderly patients undergoing hip surgery,which can maintain the stability of patients’intraoperative vital signs,reduce postoperative pain,and reduce the occurrence of intraoperative block insufficiency,so it is worth using.
