摘要
目的:探讨骨化三醇胶丸+碳酸钙D_(3)对甲状腺全切术后患者甲状旁腺功能及钙磷水平代谢的影响。方法:在九江市第一人民医院2019年1月至2020年12月收治的行甲状腺全切术的甲状腺癌患者120例,按照随机原则对其进行分组,对照组采用左甲状腺素钠+碳酸钙D_(3)治疗,研究组在其基础上联用骨化三醇胶丸治疗,对比两组甲状旁腺功能指标、生化指标、临床疗效及不良反应。结果:治疗前两组全段甲状旁腺激素、碱性磷酸酶水平、钙、磷、镁、1,25二羟基维生素D_(3)水平均对比,差异无统计学意义(P>0.05),治疗后观察组甲状旁腺两指标水平均低于对照组(P<0.05),钙、磷、镁、1,25二羟基维生素D_(3)水平均高于对照组(P<0.05);观察组总有效率高于对照组(P<0.05),两组不良反应发生对比,差异无统计学意义(P>0.05)。结论:在甲状腺全切术后患者中联合应用骨化三醇胶丸和碳酸钙D_(3)有助于促进甲状腺功能恢复,对于钙磷的补充效果显著,安全性高。
Objective:To investigate the effects of calcitriol capsules+calcium carbonate D_(3) on parathyroid function and calcium and phosphorus metabolism in patients after total thyroidectomy.Methods:120 patients with thyroid cancer who underwent total thyroidectomy in Jiujiang First People's Hospital from January 2019 to December 2020 were randomly divided into two groups.The control group was treated with sodium levothyroxine+calcium carbonate D_(3),and the study group was treated with calcitriol capsule.The parathyroid function indexes,biochemical indexes,clinical efficacy and adverse reactions of the two groups were compared.Results:There was no difference in the levels of parathyroid hormone,alkaline phosphatase,calcium,phosphorus,magnesium and 1,25 dihydroxyvitamin D_(3) between the two groups before treatment(P>0.05).After treatment,the levels of parathyroid indexes in the observation group were lower than those in the control group(P<0.05),and the levels of calcium,phosphorus,magnesium and 1,25 dihydroxyvitamin D_(3) in the observation group were higher than those in the control group(P<0.05);the total effective rate of the observation group was higher than that of the control group(P<0.05),and there was no difference in adverse reactions between the two groups(P>0.05).Conclusion:The combined application of calcitriol capsules and calcium carbonate D_(3) in patients after total thyroidectomy is helpful to promote the recovery of thyroid function.It has significant effect and high safety for calcium and phosphorus supplement.
