摘要
目的了解昆明市贫困县区(东川区、禄劝县、寻甸县)15~69岁城乡居民健康素养现状及其影响因素,为提升健康素养水平和健康扶贫政策提供指导意见。方法采用分层多阶段随机抽样方法,在昆明市贫困县区16个乡镇抽取非集体居住15~69岁的常住人口1642人进行素养监测,监测结果单因素分析采用χ2检验,多因素分析采用Logistic回归方法。结果2018年昆明市贫困县区居民健康素养水平为18.50%。基本知识和理念、健康生活方式与行为和健康技能素养水平分别为41.50%、19.24%和11.06%。居民6类健康问题素养水平由高到低依次为安全与急救、科学健康观、传染病防治、健康信息、基本医疗、慢性病防治素养水平分别为42.18%、38.96%、23.38%、21.04%、20.90%、17.24%。Logistic回归显示,年龄、文化程度、婚姻状况是昆明市贫困县区居民健康素养水平的主要影响因素(P<0.01或P<0.05),年龄(35~44岁,OR=1.888,95%CI:1.034~3.448);文化程度(高中/职高/中专,OR=2.995,95%CI:1.705~5.260;大专/本科,OR=3.310,95%CI:1.414~7.746);婚姻状况(在婚,OR=0.259,95%CI:0.086~0.776)。结论昆明市贫困县区居民健康素养水平有待提高,以中老年人、低学历者和婚姻状况以未婚人群以外人群为重点干预对象,以慢性病防治和健康信息等为重点领域开展宣传教育。
Objective To understand the status and influencing factors of health literacy among urban and rural residents aged 15-69 years old in poor counties(Dongchuan district,Luquan county and Xundian county)of Kunming city,so as to provide guidance for improving health literacy level and health poverty alleviation policy.Methods Using stratified multistage random sampling method,1642 non collective residents aged 15-69 were selected from 16 villages and towns in poor counties and districts of Kunming city for literacy monitoring.Univariate analysis was conducted with 2 test,and multivariate analysis was conducted with logistic regression method.Results In 2018,the health literacy level of residents in poor counties and districts of Kunming city was 18.50%.The literacy level of basic knowledge and concept,healthy lifestyle and behavior and health skills were 41.50%,19.24%and 11.06%respectively.From high to low,the literacy levels of 6 types of health problems were safety and first aid,scientific health concept,prevention and control of infectious diseases,health information,basic medical treatment and prevention and control of chronic diseases,which were42.18%,38.96%,23.38%,21.04%,20.90%and 17.24%respectively.Logistic regression showed that age(35-44 years old,OR=1.888,95%CI:1.034-3.448),education level(senior high school/vocational high school/technical secondary school,OR=2.995,95%CI:1.705-5.260;junior college/undergraduate,OR=3.310,95%CI:1.414-7.746)and marital status(in marriage,OR=0.259,95%CI:0.086-0.776)were the main influencing factors of health literacy level of residents in poor counties of Kunming city(P<0.01 or P<0.05).Conclusion The health literacy level of residents in poverty-stricken counties and districts of Kunming city needs to be improved,the middle-aged and the elderly,the people with low education background and whose marital status is except for the unmarried population are targeted for intervention,and the prevention and control of chronic diseases and health information as the key areas to carry out publicity and education.
