摘要
政府对公立医院的补偿政策是决定医疗卫生领域发展的重要制度安排。政府为公益目标对公立医院行政干预影响到医院的运营效率,决定了政府是财政补偿的责任主体。公立医院的社会效益与政府合法性正相关。然而,中国独特的压力型体制所要求的经济绩效和政府合法性要求的福利绩效之间的矛盾,是当前政府在执行对公立医院补偿政策过程中所遭遇的难以突破的瓶颈。这需要完善相关法律法规,加强政府财政补偿部门拨付效率的精准考核,以自下而上的审查代替评议,提高民众参与度,推进医院法人治理等措施来化解公立医院财政补偿的困境。
The government’s compensation policy for public hospitals is an important institutional arrangement to determine the development of medical and health fields.The government’s intervention in public hospitals for public welfare affects the efficiency of hospital operation determines that the government is the main body responsible for financial compensation.Its root is that the social benefits of public hospitals are positively related to the legitimacy of government.However,the contradiction between the economic performance required by China’s unique pressure system and the welfare performance required by the government’s legitimacy is a difficult bottleneck encountered by the government in the process of implementing the compensation policy for public hospitals.In order to resolve this paradox,we must according to the formation logic of the paradox,perfect the relevant laws and regulations strengthen the accurate assessment of the allocation efficiency of the government financial compensation department,replace the evaluation with the bottom-up review,and improve the participation of the public,promote the structure of hospital corporate governance,so as to fundamentally solve the internal obstacles of government finance compensation to public hospitals.
