摘要
肛周脓肿与肛瘘是同一疾病过程两个不同阶段,前者属于急性表现,以肛周肿块为主。彻底了解疾病的解剖类型与病因病机对于肛周脓肿类型诊断和治疗至关重要。如今随着磁共振、超声等影像学技术的发展,肛周脓肿的具体诊断也变得明确。治疗上可分为中医药治疗与西医治疗,前者包括内治法与外治法;后者有非手术治疗与手术治疗。手术治疗虽然可以先予切开引流,再予二期根治,但有潜在的复发率与成瘘率;一期根治术式虽能大大提高治愈率,但需与肛门功能障碍之间取得平衡;因此,保护肛门功能的保留括约肌术式又成新一热点。虽然没有单一治疗方法适合所有肛周脓肿患者,但根据不同患者病情选择合适的治疗方法应能获得更高治愈率。本文就近些年肛周脓肿相关中西医诊疗现状予以综述。
Perianal abscess and anal fistula are two different stages of the same disease process.The former is an acute manifestation,mainly perianal masses.A thorough understanding of the anatomical type and etiology and pathogenesis of the disease is essential for the diagnosis and treatment of perianal abscess types.Nowadays,with the development of imaging techniques such as MRI and ultrasound,the specific diagnosis of perianal abscess has become clear.Treatment can be divided into traditional Chinese medicine treatment and western medicine treatment.The former includes internal treatment and external treatment;the latter includes non-surgical treatment and surgical treatment.Although surgical treatment can be performed with incision and drainage first,followed by a second-stage radical treatment,it has a potential recurrence rate and fistula rate;although the first-stage radical operation can greatly improve the cure rate,it needs to be balanced with anal dysfunction.Therefore,the sphincter-preserving surgery to protect anal function has become a new hot spot.Although there is no single treatment method suitable for all patients with perianal abscess,choosing the appropriate treatment method according to the condition of different patients should achieve a higher cure rate.This article reviews the current status of Chinese and Western medicine diagnosis and treatment of perianal abscess in recent years.
