摘要
目的:通过分析上颌阻生尖牙患者的信息及锥形束CT影像学表现,来研究上颌埋伏尖牙引起邻牙牙根外吸收的危险因素,从而为临床上预测牙根外吸收提供理论依据。方法:选取在首都医科大学宣武医院口腔科拍摄锥形束CT的106例上颌埋伏尖牙病例为研究对象,其中引起邻牙牙根外吸收42例,未吸收64例,分析其锥形束CT影像学表现及患者资料。为了评价上颌阻生尖牙引起邻牙牙根外吸收的危险因素。所有的锥形束CT资料在软件中的分析判读由同一观察者完成。将所有资料分为牙根外吸收组和未吸收组,首先对两组中8项变量进行单因素分析,筛选出的危险因素代入Logistic回归方程进行分析,从而得出独立危险因素。结果:单因素分析中,性别、阻生尖牙深度、尖牙方向、尖牙牙根发育阶段、尖牙与邻牙牙根关系及尖牙牙囊情况与邻牙牙根吸收有显著相关性。Logistic回归分析结果显示,阻生尖牙的深度、阻生尖牙与邻牙牙根关系及阻生尖牙牙囊大小(P<0.01)是最主要的危险因素。其中高位相对于中位、接触组相对于未接触组及阻生尖牙牙囊增大组相对于正常牙囊的OR(优势比)值分别为:5.280、4.132及4.532。结论:高位的阻生尖牙、尖牙与邻牙牙根接触以及阻生尖牙牙囊增大是引起邻牙牙根外吸收的主要危险因素。
Objective To Linvestigate the risk factors for impacted canine-associated root resorption(ICARR) and to predict ICARR by analyzing the CBCT images and patients’ information. Methods Cone-beam computed tomography(CBCT) scan images of 106 patients with impacted maxillary canines in Xuanwu Hospital,Capital Medical University were collected. All cone-beam computed tomography data and the patients’ information were evaluated in software program by 1 rater.To identify risk factors for ICARR, the subjects with impacted canine were divided into 2 groups(with and without root resorption).Measurements of 8 variables were individually compared between the groups and then tested together by using binary logistic regressions. Results Individually, variables reflecting the depth of the impacted maxillary canine,the direction of the impacted maxillary canine,proximity between canine and adjacent root,the canine development stage,sex and the impacted canine with wide follicles(d≥2 mm)were associated with ICARR.Combined,the depth of the impacted canine(high/medium OR=5.280),proximity between canine and adjacent root(yes/no OR=4.132),the impacte canine with wide foliicles(OR=4.532) had a high risk to cause ICARR significant. Conclusion High impacted canine、proximity between canine and adjacent root and the impacted canine with wide foliicles are the most important risk fatcors for root resorption.
