摘要
目的了解气管插管急诊手术患者术后下呼吸道感染的危险因素及病原菌特点。方法通过病原菌分离鉴定技术和统计分析方法,对某医院住院行气管插管患者急诊手术后下呼吸道感染及病原菌特点进行分析。结果共调查698例气管插管急诊手术患者,术后发生下呼吸道感染患者48例,感染率为6.88%。从48例感染患者送检标本中共培养出病原菌59株,含40株革兰阴性菌、17株革兰阳性菌和2株真菌。多因素分析提示,高龄、吸烟史、误吸、留置胃管以及气管插管≥2次是术后下呼吸道感染的危险因素。结论气管插管急诊手术患者术后下呼吸道感染率较高,致病菌以革兰阴性菌为主,影响因素明确,应有针对性地制订有效防控措施。
Objective To understand the risk factors and the characteristics of pathogenic bacteria of lower respiratory tract infections in patients undergoing emergency tracheal intubation surgery.Methods Through the isolation and identification technology of pathogenic bacteria and statistical analysis methods,the characteristics of pathogenic bacteria of lower respiratory tract infection were analyzed in hospitalized patients after emergency tracheal intubation surgery in a hospital.Results A total of 698 patients undergoing emergency surgery for tracheal intubation were investigated.48 patients had lower respiratory tract infections after the operation.The infection rate was 6.88%.A total of 59 strains of pathogenic bacteria were cultured from the specimens submitted by 48 infected patients,including 40 strains of Gram-negative bacteria,17 strains of Gram-positive bacteria and 2 strains of fungi.Multivariate analysis suggested that advanced age,smoking history,aspiration,indwelling gastric tube,and tracheal intubation≥2 times were the risk factors for lower respiratory tract infection after surgery.Conclusion The infection rate of lower respiratory tract is higher in patients undergoing emergency surgery for tracheal intubation.The main pathogenic bacteria are Gram-negative bacteria.The influencing factors are clear,and effective prevention and control measures should be formulated.
