增强CT联合增强磁共振成像对乏血供肾占位良恶性的诊断价值 被引量:1

The Value of Contrast Enhanced CT Combined with Contrast Enhanced Magnetic Resonance Imaging in the Diagnosis of Benign and Malignant Space-occupying Kidneys with Poor Blood Supply

摘要 目的:探究增强CT (CECT)联合增强磁共振成像(CEMRI)鉴别诊断乏血供肾占位良恶性的价值。方法:连续性纳入2020年1—12月在惠阳三和医院收治并经手术病理结果证实的乏血供肾占位患者,均接受CECT和CEMRI检查,采用双盲法分析图像并与病理结果对照,比较分析CECT、CEMRI和CECT联合CEMRI鉴别诊断乏血供肾占位良恶性的价值。结果:共纳入63例患者的68个病灶,其中恶性和良性病灶分别有39和29个,CECT和CEMRI诊断的乏血供肾占位的敏感度、特异度分别为71.79%和79.31%、79.49%和89.66%,CECT联合CEMRI诊断的敏感度、特异度和准确度分别为97.44%、93.10%。CECT联合CEMRI诊断乏血供肾占位良恶性的价值高于单独CECT、CEMRI检查(χ^(2)=16.02、8.08,P<0.05)。结论:CECT联合CEMRI成像有助于提高乏血供肾脏占位良恶性的诊断准确性。 Objective:To investigate the value of contrast enhanced CT(CECT) combined with contrast enhanced magnetic resonance imaging(CEMRI) in diagnosing benign and malignant hypovascular renal lesions.Methods:Patients with hypovascular renal lesions who were admitted to the hospital from January 2020 to December 2020 were continuously included,all of them received enhanced CECT and CEMRI examinations.Double-blind method was used to analyze images and to compare with pathological results.The value of enhanced CECT,CEMRI,and enhanced CECT combined with CEMRI in the differential diagnosis of benign and malignant space-occupying kidneys with insufficient blood supply were compared and analyzed.Results:A total of 68 lesions from 63 patients were included,of which 39 were malignant and 29 were benign.The sensitivity and specificity of enhanced CECT and CEMRI diagnosis of poor blood supply kidney occupying areas were 71.79% and 79.31%,79.49% and 89.66%.The sensitivity,specificity and accuracy of enhanced CECT combined with CEMRI were 97.44% and 93.10%,respectively.The value of enhanced CECT combined with CEMRI in diagnosing benign and malignant space-occupying kidneys with insufficient blood supply was higher than that of enhanced CECT and CEMRI alone(χ^(2)=16.02,8.08,P<0.05).Conclusion:Enhanced CECT combined with CEMRI imaging can help improve the diagnostic accuracy of benign and malignant kidneys with poor blood supply.
