摘要
目的观察评价在鼻窦炎鼻息肉治疗中施行鼻内镜手术与鼻腔开放手术的临床效果及有效性。方法选择2019年1月—2020年10月该院收治的120例鼻窦炎鼻息肉患者为研究对象。根据数字抽取法进行分组,对照组(60例)实施鼻腔开放手术,与研究组(60例)实施鼻内镜手术的临床总有效率、症状改善情况、嗅觉功能、鼻气道阻力与鼻腔最小横截面面积(NMCA)、鼻腔容积(NCV)等各项指标进行对比。结果从临床疗效分析,研究组治疗总有效率96.67%(58/60)较对照组75.00%(45/60)明显更高,差异有统计学意义(χ^(2)=11.582,P=0.001)。从病情恢复分析,术前,研究组患者的NMCA(6.43±1.44)cm^(3)、NCV(0.23±0.06)cm^(3)对比对照组(6.42±1.43)cm^(3)、(0.22±0.07)cm^(3)差异无统计学意义(P>0.05);术后,研究组患者的NMCA(13.46±2.72)cm^(3)、NCV(0.72±0.11)cm^(3)对比对照组(10.98±2.04)cm^(3)、(0.54±0.08)cm^(3)更高,差异有统计学意义(P<0.05)。术前,研究组患者的嗅觉功能(3.08±0.31)kPa/sL、鼻气道阻力(2.65±0.85)分与对照组(3.06±0.36)kPa·s/L、(2.67±0.86)分比较差异无统计学意义(P>0.05);术后,研究组患者的嗅觉功能(0.29±0.02)kPa·s/L、鼻气道阻力(6.95±0.98)分与对照组(0.49±0.16)kPa·s/L、(5.19±0.47)分相比更低,差异有统计学意义(P<0.05);在症状恢复效果评价中,术前,两组患者的疼痛、鼻塞、嗅觉障碍、脓涕等各项症状评分对比差异无统计学意义(P>0.05);术后,研究组的上述各项症状评分较对照组更低,差异有统计学意义(P<0.05)。结论针对临床罹患鼻窦炎鼻息肉患者,采取鼻内镜手术治疗效果显著,可促进其鼻腔结构得以快速恢复,同时可有效改善其不适症状与气道阻力,增强嗅觉功能,优化疾病治疗效果。
Objective To observe and evaluate the clinical effects and effectiveness of endoscopic sinus surgery and open nasal cavity surgery in the treatment of sinusitis and nasal polyps.Methods The 120 patients with sinusitis and nasal polyps admitted to the hospital from January 2019 to October 2020 were selected as the research objects.Grouped according to the digital extraction method,the control group(60 cases)performed open nasal cavity surgery,and the study group(60 cases)performed nasal endoscopic surgery.The total clinical effectiveness,symptom improvement,olfactory function,nasal airway resistance and minimal nasal cavity Cross-sectional area(NMCA),nasal cavity volume(NCV)and other indicators are compared.Results From the analysis of clinical efficacy,the total effective rate of treatment in the study group was 96.67%(58/60)than that of the control group at 75.00%(45/60),and the difference was statistically significant(χ^(2)=11.582,P=0.001).From the recovery analysis.before surgery,there was not statistically significant difference in NMCA(6.43±1.44)cm^(3) and NCV(0.23±0.06)cm^(3) of the study group compared with the control group(6.42±1.43)cm^(3) and(0.22±0.07)cm^(3)(P>0.05),Later,the NMCA(13.46±2.72)cm^(3) and NCV(0.72±0.11)cm^(3) of the study group were higher than those of the control group(10.98±2.04)cm^(3) and(0.54±0.08)cm^(3),and the difference was statistically significant(P<0.05).Before surgery,there was not statistically significant difference in the olfactory function(3.08±0.31)kPa·s/L and nasal airway resistance(2.65±0.85)points of the study group compared with the control group(3.06±0.36)kPa·s/L and(2.67±0.86)points(P>0.05),after the operation,the olfactory function(0.29±0.02)kPa·s/L and nasal airway resistance(6.95±0.98)points of the study group were compared to the control group(0.49±0.16)kPa·s/L,(5.19±0.47)points was lower,and the difference was statistically significant(P<0.05).In the evaluation of the symptom recovery effect,before the operation,the statistical values of the pain,nasal congestion,olfactory dysfunction,purulent discharge and other symptoms of the two groups of patients were compared,the difference was not statistically significant(P>0.05);after the operation,the scores of the above-mentioned symptoms in the study group were lower than those in the control group,and the difference was statistically significant(P<0.05).Conclusion For clinical patients suffering from sinusitis and nasal polyps,endoscopic sinus surgery has a significant effect,which can promote the rapid recovery of the nasal cavity structure,and can effectively improve the symptoms of discomfort and airway resistance,enhance the sense of smell,and optimize the treatment effect of the disease.
