经颅多普勒超声发泡实验在卵圆孔未闭诊断及封堵疗效评价中的价值 被引量:11

摘要 目的:探究经颅多普勒超声发泡实验(contrast transcranial Doppler,c-TCD)在卵圆孔未闭(patent foramen ovale,PFO)诊断及封堵疗效评价中的价值。方法:选取经c-TCD筛查为Ⅳ级阳性的隐源性卒中/短暂性脑缺血发作或偏头痛患者145例,采用经食管超声心动图(transesophageal encocardiography,TEE)检查。对于有经导管介入封堵治疗适应证的患者实行介入手术,并对术后患者进行随访。结果:c-TCD筛查为Ⅳ级阳性的隐源性卒中/短暂性脑缺血发作或偏头痛患者中,144例经导管介入明确为PFO。在这144例确诊患者中,TEE可明确诊断的有115例(79.9%);其中对于右向左分流(right-to-left shunt,RLS)固有型的106例患者,同时可经TEE观察到PFO的有87例,诊断敏感性为82.1%,而对于RLS潜在型的38例患者,同时可经TEE观察到PFO的有28例,诊断敏感性为73.7%。统计学分析提示,这两组中TEE检查的阳性率差异无统计学意义(χ^(2)=1.225,P=0.268),表明存在RLS的PFO,无论其RLS发生在静息下还是Valsalva动作时,TEE的诊断敏感性没有差异。二元Logistic回归分析提示PFO封堵治疗后c-TCD检查为大量分流是患者术后症状不缓解的独立危险因素(P<0.05)。结论:c-TCD可用于隐源性卒中/短暂性脑缺血发作或偏头痛患者是否合并PFO的病因筛查,对于接受经导管介入封堵治疗的PFO患者,c-TCD则是较好的疗效评价手段。
