摘要
目的探讨急性缺血性脑卒中肇事血管CT值与血管再通效果的相关性。方法收集2019年1月-2020年3月间,在我院行溶栓及血管腔内治疗的急性脑动脉主干闭塞患者58例,在初诊CT平扫及增强扫描图像上测量肇事血管闭塞处管腔的CT值,并测量对侧或邻近通畅主干血管腔的CT值为参照,以治疗后闭塞血管再通较好的为A组、管腔再通不理想的为B组,分析肇事血管闭塞处CT值与血管再通之间的关系。结果患者发病至溶栓平均时间约153 min。治疗后评估,A组与B组的血管闭塞处平扫CT值比较[(60.78±11.03) HU vs (32.20±7.17) HU,P <0.05]、通畅血管腔平扫CT值比较[(42.53±6.92) HU vs (42.45±3.75) HU,P>0.05]。结论急性缺血性脑卒中患者中,肇事血管闭塞处CT值较高者,闭塞管腔再通效果较好。
Objective To explore the correlation between CT value of infarcted vessel and the effect of vascular recanalization in acute ischemic stroke. Methods Fifty-eight patients with acute cerebral arterial trunk occlusion treated by endovascular therapy in our hospital from January 2019 to March 2020 were collected. The CT value of the infarcted vessel was measured on the plain and enhanced CT images while diagnosis,and the CT value of the contralateral or adjacent unobstructed main vessel was measured as a reference. Patients with better recanalization of occluded vessels after treatment were set as group A,and patients with unsatisfactory recanalization of occluded vessels were set as group B. The relationship between the CT value of the occluded vessel and the recanalization of the vessel was analyzed. Results The average time from onset to thrombolysis was about153 min. After treatment,the plain scan CT value of vascular occlusion position was compared between Group A and Group B[( 60.78 ± 11.03) HU vs( 32. 20 ± 7. 17) HU,P < 0. 05],and the CT value of nonoccluded vascular lumen was compared[( 42.53 ±6.92) HU vs( 42.45 ±3.75) HU,P >0. 05]. Conclusion In patients with acute ischemic stroke,the recanalization effect of occluded vessel is better in patients with higher CT value at the site of vascular occlusion.
