摘要
1例43岁女性患者,因“胸闷、胸痛2个月余”入院,入院后完善相关检查及穿刺活检,诊断为原发性心脏血管肉瘤伴双肺转移。由于该病较为罕见,且恶性程度高、进展快、预后差,故目前尚无统一的治疗方案。经查阅相关文献及指南后,决定为患者行化疗联合免疫治疗,治疗方案为紫杉醇(白蛋白结合型)+卡铂+帕博利珠单抗,期间未出现明显不良反应,2个周期后复查提示病灶稳定未见明显进展,目前仍在临床随访中,无进展生存期6个月。
A 43-year-old female patient was admitted to the hospital because of chest tightness and chest pain for more than 2 months.After admission,the relevant examinations and needle biopsy were completed and she was diagnosed with primary cardiac angiosarcoma with metastases in both lungs.Because primary cardiac angiosarcoma is a rare disease with a high degree of malignancy,rapid progress,and poor prognosis,there is no unified treatment regimen at present.After consulting relevant literature and guidelines,it was decided to give chemotherapy combined with immunotherapy to the patient.The treatment regimen was paclitaxel(albumin binding),carboplatin and pembrolizumab.There were no obvious adverse drug reactions during the period of treatment.Reexamination after 2 cycles indicated that the lesion was stable without significant progress.The patient is still in clinical follow-up and the progression free survival time is 6 months.
