摘要
目的:分析内镜下鼻中隔成形术联合鼻骨骨折复位术对鼻骨骨折伴鼻中隔骨折患者的美学效果。方法:将笔者医院耳鼻喉科2017年1月-2019年12月收治的54例鼻骨骨折伴鼻中隔骨折患者随机分为对照组(27例)和观察组(27例)。对照组采用内镜下鼻骨骨折复位术进行治疗,观察组在对照组的基础上联合鼻中隔成形术治疗,两组术后均随访6个月。比较两组术前、术后6个月视觉模拟评分(VAS)、鼻腔通气功能、鼻外形优良率和住院期间并发症发生情况。结果:与术前比较,术后6个月两组VAS评分、鼻腔呼气吸气阻力均降低,观察组低于对照组,差异有统计学意义(P<0.05),两组鼻腔最小横截面积、鼻腔容积、鼻总呼气量增大,观察组高于对照组(P<0.05)。术后6个月,观察组鼻外形优良率为92.59%,显著高于对照组70.37%(P<0.05)。住院期间,观察组总并发症发生率与对照组比较,差异无统计学意义(P>0.05)。结论:内镜下鼻中隔成形术联合鼻骨骨折复位术治疗鼻骨骨折伴鼻中隔骨折,可有效改善患者鼻通气情况和鼻外形,提高面部美学效果,同时术后并发症少,具有良好的治疗效果。
Objective To analyze the aesthetic effect of endoscopic septoplasty combined with reduction of nasal bone fracture in patients with nasal bone fracture and septal fracture.Methods The 54 patients with nasal bone fractures and septal fractures who were admitted to the Department of Otolaryngology at the author's hospital from January 2017 to December 2019 were randomly divided into a control group(27 cases)and an observation group(27 cases).The control group was treated by reduction of nasal bone fracture,while the observation group was treated by endoscopic septoplasty combined with reduction of nasal bone fracture.Both groups were followed up for 6 months after operation.The visual analogue size(VAS)and nasal ventilation function of the two groups were compared before and 6 months after operation,the complications during hospitalization and good rate of nasal appearance six months after operation were counted.Results Compared with preoperatively,the VAS score and nasal exhalation and inspiratory resistance of the two groups were reduced at 6 months after the operation.The observation group was lower than that of the control group.The difference was statistically significant(P<0.05).The total nasal exhalation volume increased,and the observation group was higher than that of the control group(P<0.05).At 6 months after operation,the excellent rate of nasal shape in the observation group was 92.59%,which was significantly higher than 70.37%in the control group(P<0.05).During the hospitalization period,there was no statistically significant difference in the total complication rate between the observation group and the control group(P>0.05).Conclusion Endoscopic nasal septum plasty combined with reduction of nasal bone fracture can effectively improve nasal ventilation and nasal contour,and enhance facial aesthetic effect,with few postoperative complications and good therapeutic effect.
