静脉-静脉体外膜肺氧合联合俯卧位通气治疗重度急性呼吸窘迫综合征临床研究进展 被引量:10

Clinical research progress of veno-venous extracorporeal membrane oxygenation combined with prone position ventilation in the treatment of severe acute respiratory distress syndrome

摘要 尽管近年来对急性呼吸窘迫综合征(ARDS)的临床认识和治疗研究取得了一定进展,但目前ARDS在重症医学科(ICU)内的病死率仍居高不下,静脉-静脉体外膜肺氧合(V-VECMO)联合俯卧位通气作为治疗重度ARDS的关键措施,其作用机制以及对肺损伤的协同保护作用已被临床医师逐渐认识,并逐步在临床应用。但在重度ARDS治疗过程中体外膜肺氧合(ECMO)最佳启动时间、ECMO期间呼吸机最佳设置、ECMO启动后至第一次俯卧位通气时间以及ECMO期间俯卧位通气持续时间等仍缺乏统一认识,本文就V-V ECMO联合俯卧位通气的相关问题研究进展作一综述。 Although some progress has been made in the clinical understanding and treatment of acute respiratory distress syndrome(ARDS)in recent years,the mortality rate of ARDS in ICU is still high,veno-venous extracorporeal membrane oxygenation(V-V ECMO)combined with prone position ventilation is the key measure for the treatment of severe ARDS,its mechanism and synergistic protective effect on injured lung have been gradually recognized by clinicians and gradually applied in clinical practice.However,in the treatment of severe ARDS,there is still a lack of unified understanding of the optimal start time of ECMO,the optimal setting of ventilator during ECMO,the time from the start of ECMO to the first prone position ventilation and the duration of prone position ventilation during ECMO.This article will review the research progress of V-V ECMO combined with prone position ventilation.
