摘要
目的探讨缩宫素联合蛇毒血凝酶对产后出血产妇氧化应激、血流动力学及安全性的影响。方法选取湖北省洪湖市人民医院2019年3月至2020年3月收治的产后出血产妇90例,按随机数字表法分为观察组和对照组,各45例。两组产妇均予缩宫素治疗,观察组加用蛇毒血凝酶治疗,均连续治疗3 d。结果观察组产妇分娩后2 h、24 h出血量和出血率均显著低于对照组(P<0.05);两组产妇术后24 h丙二醛水平均显著低于手术结束时,超氧化物歧化酶、谷胱甘肽过氧化物酶、过氧化氢酶水平均显著高于手术结束时,且观察组改善更显著(P<0.05);术后2 h两组产妇心率显著高于术前,收缩压和舒张压均显著低于术前,且观察组改善更显著(P<0.05);术前及术后2 h两组产妇血氧饱和度差异不显著(P>0.05);观察组产妇不良反应发生率为6.67%,显著低于对照组的28.89%(P<0.05)。结论缩宫素联合蛇毒血凝酶用于产后出血能有效降低产妇的出血量及出血率,减轻氧化还原应激反应,维持血流动力学稳定,降低术后并发症发生率。
Objective To investigate the effects of oxytocin combined with snake venom hemagglutinase on oxidative stress,hemodynamics and safety in parturients with postpartum hemorrhage.Methods A total of 90 parturients with postpartum hemorrhage admitted to the Honghu People's Hospital from March 2019 to March 2020 were selected and divided into the observation group and the control group according to the random number table method,with 45 cases in each group.The parturients in the two groups were treated with oxytocin,on this basis,the parturients in the observation group were treated with snake venom hemagglutinase.Both groups were continuously treated for 3 d.Results The amount and rate of postpartum hemorrhage at 2 h and 24 h after delivery in the observation group were were significantly lower than those in the control group(P<0.05).At 24 h after operation,the level of malondialdehyde(MDA)in the two groups was significantly lower than that at the end of operation,while the levels of superoxide dismutase(SOD),glutathione peroxidase(GSH-Px)and catalase(CAT)in the two groups were significantly higher than those at the end of operation,and the improvement in the observation group was more significant(P<0.05).At 2 h after operation,the heart rate(HR)in the two groups was significantly higher than that before operation,while the systolic blood pressure(SBP),diastolic blood pressure(DBP)in the two groups were significantly lower than that before operation,and the improvement in the observation group was more significant(P<0.05).There was no significant difference in blood oxygen saturation(SpO_(2))between the two groups before operation and at 2 h after operation(P>0.05).The incidence of adverse reactions in the observation group was 6.67%,which was significantly lower than 28.89%in the control group(P<0.05).Conclusion Oxytocin combined with snake venom hemagglutinase can effectively reduce the amount and rate of postpartum hemorrhage,reduce oxidative stress,maintain hemodynamic stability,and reduce the incidence of postoperative complications.
