小野对动态调强放疗计划剂量分布的影响

摘要 目的探讨小野对动态调强放疗计划剂量分布的影响。方法随机选取2020年医院放疗技术科的20例肿瘤患者的动态调强放疗计划,分别采用含实测小野数据的AAA算法和不含实测小野数据的AAA算法计算其剂量分布,比较两种算法的通过率。结果以3 mm/3%为标准,含实测小野数据的AAA算法通过率均高于不含实测小野数据的AAA算法,对两种算法的计划验证结果进行配对t检验,差异有统计学意义(P<0.05)。结论在动态调强放疗计划中多叶准直器连续运动所形成的小野对剂量分布产生了影响,有条件的情况下建议实测加速器的小野剂量学数据。 Objective The effect of small field on dose distribution in dynamic intensity modulated radiotherapy plan was investigated. Methods The dynamic intensity modulated radiotherapy plans of 20 patients in the Department of Radiotherapy Technology of the hospital in 2020 was randomly selected, the AAA algorithms with and without measured data of small field were both used to calculate the dose distribution, and the pass rates of the two algorithms were compared.Results The pass rate of the AAA algorithm with measured data of small field were all higher than those of the AAA algorithm without measured data of small field with the standard of 3 mm/3%. There was a significant difference between the two algorithms in the results of paired t-test(P<0.05). Conclusion The small fields formed by the continuous movement of the multi-leaf collimator in the dynamic intensity modulated radiotherapy plan have an effect on the dose distribution. If possible, it is recommended to measure the small field dosimetry data of the accelerator.
