渐进式弹力带抗阻运动对老年2型糖尿病合并肌少症患者糖脂代谢的影响 被引量:4

Effects of progressive elastic band resistance exercise on glucose and lipid metabolism in elderly patients with type 2 diabetes and sarcopenia

摘要 目的:探究渐进式弹力带抗阻运动应用于2型糖尿病(Diabetes mellitus type 2,T2MD)合并肌少症老年患者的影响。方法:纳入本科2018年8月至2020年8月T2MD合并肌少症老年患者120例,按照随机数字表法分为两组(n=60),对照组患者有氧运动训练,观察组患者联合渐进式弹力带抗阻运动,治疗3 m后采用血红蛋白分析仪及血糖仪分别测定糖化血红蛋白(Hemoglobin A1C,HbA1c)和空腹血糖(Fasting blood-glucose,FPG)水平;采用全自动生化仪测定三酰甘油(Triacylglycero,TG)、总胆固醇(Total Cholesterol,TC)及低密度脂蛋白胆固醇(Low density lipoprotein cholesterol,LDL-C)水平;使用化学发光免疫分析仪检测血清胰岛素(Fasting serum lisulin,FINs)水平,计算算胰岛素抵抗指数(homeostasis model assessment of insulin resistance,HOMA-IR);使用握力计测量优势手握力;记录6m步行速度;使用人体分析仪检测四肢骨骼肌质量指数(Skeletal muscle index,SMI)水平;使用日常活动能力(Activities of daily living,ADL)评价患者生活质量,并观察不良反应发生情况。结果:干预后,观察组HbA1c、FPG、TG、TC、LDL-C水平低于对照组(P<0.05);FINs、HOMA-IR水平较对照组低(P<0.05);握力、SMI及6 m步行速度水平高于对照组(P<0.05);ADL评分较对照组高(P<0.05);两组不良反应比较无统计学意义(P>0.05)。结论:渐进式弹力带抗阻运动对T2MD合并肌少症患者可有效辅助缓解胰岛素抵抗,恢复糖脂代谢,提高肌力和生活质量,安全可靠。 Objective:To explore the effects of progressive elastic band resistance exercises in elderly patients with type 2 diabetes(Diabetes mellitus type 2,T2MD)complicated with sarcopenia.Methods:A total of 120 elderly patients with T2MD combined with sarcopenia from August 2018 to August 2020 were enrolled in the undergraduate.All patients were divided into two groups(n=60)according to the random number table method.The patients in control group were received aerobic exercise training,and the patients in observation group were received aerobic exercise training combined progressive elastic band resistance exercise.After 3 m,hemoglobin analyzer and blood glucose meter were used to measure glycosylated hemoglobin(HbA1c)and fasting blood-glucose(FPG)levels,and automatic biochemical analyzer was used to measure triacylglycerol(TG),total cholesterol(TC)and low density lipoprotein cholesterol(LDL-C)levels,chemiluminescence immunoassay was used to detect serum fasting serum lisulin(FINs)levels and calculate the homeostasis model was used to assess insulin resistance(HOMA-IR)level,a grip meter was used to measure the dominant hand grip strength;6 m walking speed was recorded;a human body analyzer was used to detect the skeletal muscle index(SMI)of the extremities level,activities of daily living(ADL)was used to evaluate the patient’s quality of life and adverse reactions were recorded.Results:After the intervention,the levels of HbA1c,FPG,TG,TC,and LDL-C in the observation group were lower than those in the control group(P<0.05);the levels of FINs and HOMA-IR were lower than those in the control group(P<0.05);grip strength,SMI and 6 m level of walking speed were higher than those in the control group(P<0.05);the ADL score was higher than that in the control group(P<0.05);the adverse reactions between the two groups were not statistically significant(P>0.05).Conclusion:Progressive elastic band resistance exercise can effectively help relieve insulin resistance,restore glucose and lipid metabolism,improve muscle strength and quality of life in patients with T2MD combined with sarcopenia,which is safe and reliable.
