摘要
目的探讨慢性心衰患者血浆氨基末端脑钠肽前体(N-terminal pro-brain natriuretic peptide,NT-proBNP)在诊断和评价心衰重构进展程度中的作用及其临床应用价值。方法109例慢性心衰患者与100例正常人为研究对象,检测心衰重构患者与非心衰患者的血浆NT-proBNP,分析相关参数与血浆NT-proBNP相关性。结果心衰组与正常组比较,血浆中NT-proBNP水平显著升高[(2539.1±1095.0)pg/mL vs(485.4±255.0)pg/mL];心功相关参数:左室舒张末内径(left ventricular end diastolic inner diameter,LVEDD)[(61.6±8.3)mm vs(45.2±3.4)mm]、左室收缩末期内径(left ventricular end systolic diamete,LVESD)[(49.1±8.9)mm vs(27.9±3.2)mm]、左心房内径(left atrium diameter,LA)[(40.3±7.5)mm vs(31.4±3.0)mm]以及左心室射血分数(left ventricular ejection fraction,EF)[(39.4±5.3)mm vs(68.1±5.2)mm]均有所改变(P<0.05),其中患者的LVEDD、LVESD以及LA与血浆NT-proBNP水平呈正相关,患者的EF、FS则与血浆NT-proBNP水平呈负相关(P<0.05)。结论血浆NT-proBNP与心衰患者的发展变化具有明显相关性,可以作为心衰患者评估指标。
Objective To investigate the role of N-terminal pro-brain natriuretic peptide(NT-proBNP)in the diagnosis and evaluation of the progression of heart failure remodeling in patients with chronic heart failure and to explore its clinical application value.Methods One hundred and nine patients with chronic heart failure and 100 normal subjects were collected.The plasma NT-proBNP of patients with remodeled heart failure and non-heart failure patients were detected,and the correlation between related parameters and plasma NT-proBNP was analyzed.Results Compared with the normal group,the level of NT-proBNP in the heart failure group was significantly increased((2539.1±1095.0)pg/mL vs(485.4±255.0)pg/mL);the left ventricular end diastolic diameter,including left ventricular end diastolic inner diameter(LVEDD)((61.6±8.3)mm vs(45.2±3.4)mm),left ventricular end systolic diamete(LVESD)((49.1±8.9)mm vs(27.9±3.2)mm),left atrium diameter(LA)((40.3±7.5)mm vs(31.4±3.0)mm)and left ventricular ejection fraction(EF)((39.4±5.3)mm vs(68.1±5.2)mm)were all changed(P<0.05).Among them,patients’LVEDD,LVESD,and LA were positively correlated with plasma NT-proBNP levels,while patients EF and FS were negatively correlated with plasma NT-proBNP levels(P<0.05).Conclusion Plasma NT-proBNP has a significant correlation with the development and changes of patients with heart failure,and can be used as an evaluation index for patients with heart failure.
