摘要
目的:研究中药柴芩通淋片(chaiqintonglin,CQTL)在连续用药后对临床分离泌尿系感染大肠埃希菌耐药特性的影响。方法:首先微量稀释法分别测定柴芩通淋片及临床常用抗生素对临床分离大肠埃希菌的最小抑菌浓度(minimal inhibitory concentration,MIC),而后用1/4 MIC浓度的柴芩通淋片对20株耐药大肠埃希菌进行连续用药5代,分别检测5次用药后20株耐药大肠埃希菌对各类常用抗生素药物敏感性变化,选择前后药物敏感性变化显著的细菌观察菌落形态并进行连续10代培养,通过测定常用抗生素MIC值变化情况判断其是否具有稳定遗传特性。结果:常用抗生素MIC值测定结果表明20株菌均为多重耐药菌株,柴芩通淋片在0.24 g/mL时为20株中9株菌的最低抑制耐药菌株浓度。经过1/4 MIC柴芩通淋片对20株大肠埃希菌连续作用后,其中3株出现对头孢他啶敏感性恢复。对出现头孢他啶敏感性恢复的菌株连续空白培养10代,其MIC值未发生变化。结论:经过1/4 MIC柴芩通淋片连续作用后,可恢复部分临床大肠埃希菌对头孢他啶药物敏感性作用,其耐药性的恢复具有稳定遗传特性。
Objective:To study the effect on the drug resistance characteristics of Escherichia coli(E Coli)isolated from urinary tract infection after continuous administration of Chaiqin Tonglin(CQTL)tablet treatment.Methods:First,the minimum inhibitory concentration(MIC)of Chaiqin Tonglin tablet and common antibiotics against clinical isolates of E Coli was determined by the microdilution method.Then,20 strains of resistant E Coli were treated with Chaiqin Tonglin tablet at 1/4MIC concentrations for 5 successive generations.The sensitivity changes of 20 strains of drug-resistant E Coli to all kinds of commonly used antibiotics after 5 doses were detected.The differences in bacterial drug sensitivity after treatment were observed and then cultured for 10 consecutive generations.The differences in MIC value of commonly used antibiotics were measured to determine whether the bacteria had stable genetic characteristics.Results:The MIC values of common antibiotics showed that all the 20 strains were multi-drug resistant strains.The concentration of Chaiqin Tonglin tablet at 0.24 g/mL was the minimum inhibitory concentration of 9 strains in the 20 strains.After continuous action of 1/4MIC Chaiqin Tonglin tablet on 20 strains of E coli,3 of them showed sensitivity recovery to ceftazidime.The MIC value of ceftazidime was not changed when the strain showed sensitive recovery of ceftazidime was cultured for 10 consecutive generations.Conclusion:Partial clinical E coli sensitivity to ceftazidime can be restored after 1/4MIC Chaiqin Tonglin tablet continuous action,and the recovery of drug resistance has stable genetic characteristics.
