摘要
胃肠功能障碍是胃癌术后常见的并发症,主要表现为便秘与腹泻。本文基于病机十九条“诸厥固泄,皆属于下”探讨胃癌术后胃肠功能障碍从肾论治。胃癌手术、放化疗伤及脏腑元气,使脾胃衰弱难以恢复进而损及于肾。同时脾胃虚弱无力运化精微以滋养肾,肾气虚则大肠传导乏力,糟粕内结于里,当补肾气以通达肠道,然脾肾相互滋养,补肾亦当补脾;肾主水司二便,肾阴亏虚、津液无法正常运行生化,以致肠道津枯,大便艰涩难下,当滋肾阴以濡润肠腑;肾阳衰微,火不暖土,可致脾胃虚寒,湿浊下注于大肠泄泻不止,当益命门以涩肠止泻使胃肠功能恢复正常。本文按病机在肾、治亦在肾辨证论治,为临床胃癌术后治疗提供新的思路。
Gastrointestinal dysfunction is a common postoperative complication of gastric cancer,mainly manifested as constipation and diarrhea.In this paper,based on the article 19 of pathogenesis"all the convulsions and solid leakage belong to the lower part",the gastrointestinal dysfunction after gastric cancer operation is treated from the perspective of kidney.Gastric cancer surgery,radiotherapy and chemotherapy injury and viscera vitality,spleen and stomach weakness is difficult to recover,and then damage to the kidney.At the same time,the spleen and stomach are weak and weak to nourish the kidney.Kidney qi deficiency is the large intestine conduction weakness,dregs knot in,when tonifying kidney qi to access the intestine,but the spleen and kidney nourish each other,tonifying kidney also when tonifying spleen;Kidney water division,kidney Yin deficiency,body fluid can not normal operation of biochemical,so that intestinal jin withered,stool difficult and difficult,when nourishing kidney Yin to moisten the intestine;Kidney Yang decline,fire is not warm soil,wet turbidness placed in the large intestine,diarrhea,when Yimingmen to astringent intestinal diarrhea,gastrointestinal function to return to normal.In this paper,according to the pathogenesis of kidney and the treatment of kidney,the treatment based on syndrome differentiation provides a new idea for the postoperative treatment of gastric cancer.
