摘要
目的探讨超声引导下主干透皮缝扎联合硬化剂注射治疗大隐静脉曲张疗效。方法收集2020年2月至2021年2月阜阳市肿瘤医院治疗的73例下肢静脉曲张患者的临床资料。根据治疗方法不同分为透皮缝扎组(A组,n=32)和传统高位结扎+点式抽剥组(B组,n=41)。观察两组患者手术指标(手术时间、术中出血量、住院时间、住院费用),术后1个月,并发症(肿胀、疼痛、隐神经损伤、切口感染、血栓性静脉炎)发生率。随访6个月,超声评估两组患者大隐静脉主干完全闭塞情况。结果A组患者手术时间、住院时间短于B组患者,术中出血量、住院费用少于B组患者,差异有统计学意义(P<0.05)。术后1个月,两组患者血栓性静脉炎发生率比较,差异无统计学意义(P>0.05);A组患者无肿胀、疼痛、隐神经损伤、切口感染并发症发生,B组患者肿胀、疼痛、隐神经损伤、切口感染发生率分别为19.51%、17.07%、17.07%和17.07%,两组比较差异有统计学意义(P<0.05);术后6个月,两组患者大隐静脉主干闭合率比较,差异无统计学意义(P>0.05)。结论应用超声引导主干透皮缝扎联合硬化剂注射治疗原发性大隐静脉曲张安全、有效,住院时间短且费用低,值得临床推广。
Objective To evaluate the effect of ultrasound-guided trunk percutaneous ligation combined with foam sclerotherapy in treating varicose saphenous vein.Method The clinical data of 73 patients with varicose veins of the lower extremities treated in Fuyang Tumor Hospital from February 2020 to February 2021 were collected.According to different treatment methods,all patients were divided into the transdermal suture group(group A,n=32),traditional high ligation plus point stripping group(group B,n=41).The surgical indicators(operating time,intraoperative blood loss,hospitalization time,hospitalization expense)of the two groups of patients were observed.One month after the surgery,the incidence of complications(swelling,pain,saphenous nerve injury,incision infection,thrombophlebitis).After a 6-month follow-up,an ultrasound assessment of the complete occlusion of the great saphenous vein closure of the two groups of patients was conducted.Result The operation time and hospital stay in group A was shorter than those in group B,and the blood loss volume and hospitalization expenses during the operation were lower than those in group B.The differences were statistically significant(P<0.05).One month after the operation,there was no significant difference in the incidence of thrombophlebitis between the two groups of patients(P>0.05).There were no complications of swelling,pain,saphenous nerve injury,and incision infection reported in group A,while the incidences of swelling,pain,saphenous nerve injury,and incision infection in group B were 19.51%,17.07%,17.07% and 17.07%,respectively.And the differences between the two groups were statistically significant(P<0.05).At 6 months after the operation,there was no significant difference in the rate of the great saphenous vein closure between the two groups(P>0.05).Conclusion The employment of ultrasound-guided main trunk transdermal suture combined with polidocanol foam sclerotherapy is safe and effective in treating primary varicose veins.Besides,the hospital stay is short and the cost is low when taking this plan,indicating its value of clinical promotion.
