摘要
目的:观察止咳平喘十二味合剂治疗痰热郁肺型咳嗽变异型哮喘的临床疗效及其对肺功能和外周血免疫球蛋白E(IgE)、肿瘤坏死因子-α(TNF-α)、白细胞介素-8(IL-8)的影响。方法:将2018年9月—2020年8月间由呼吸科收治且符合研究条件的咳嗽变异型哮喘患者92例,按随机化原则分为2组各46例。对照组采用孟鲁司特钠联合多索茶碱治疗,观察组在对照组治疗方案的基础上联合止咳平喘十二味合剂治疗,2组均治疗2周,比较2组治疗前后咳嗽症状积分、咳嗽相关生活质量评分、肺功能指标及外周血IgE、TNF-α、IL-8。结果:治疗2周后,观察组咳嗽症状积分低于对照组(P<0.05);咳嗽相关生活质量评分高于对照组(P<0.05),观察组肺功能指标第1秒用力呼气容积(FEV1)、呼气峰值流速(PEF)及用力呼气中期流速(MMEF75/25)值均高于对照组(P<0.05),观察组外周血IgE、TNF-α、IL-8均低于对照组(P<0.05)。结论:止咳平喘十二味合剂可显著降低痰热郁肺型咳嗽变异型哮喘患者外周血IgE、TNF-α和IL-8等炎症因子水平,改善肺功能,减轻咳嗽症状,提高咳嗽相关生活质量。
Objective:To observe the clinical effect of Zhike Pingchuan Shierwei mixture for cough-variant asthma of phlegm-heat obstructing the lung type and its effect on the lung function as well as immunoglobulin E(IgE),tumor necrosis factor-α(TNF-α)and interleukin-8(IL-8)in peripheral blood.Methods:A total of 92 cases of patients with cough-variant asthma who were treated in the Respiratory Department from September 2018 to August 2020 and met the research conditions were randomly divided into two groups,with 46 cases in each group.The control group was treated with montelukast sodium combined with doxofylline,and the observation group was additionally treated with Zhike Pingchuan Shierwei mixture based on the treatment of the control group.Both groups were treated for two weeks.Before and after treatment,scores of cough and cough-related quality of life,the lung function indexes,as well as IgE,TNF-αand IL-8 in peripheral blood were compared between the two groups.Results:After two-week treatment,the symptom score of cough in the observation group was lower than that in the control group(P<0.05);the score of cough-related quality of life was higher than that in the control group(P<0.05);the indexes of the lung function including the forced expiratory volume in one second(FEV1),peak expiratory flow(PEF)and maximal mid-expiratory flow rate(MMEF75/25)in the observation group were higher than those in the control group(P<0.05);IgE,TNF-αand IL-8 in peripheral blood in the observation group were lower than those in the control group(P<0.05).Conclusion:For patients with cough-variant asthma of phlegm-heat obstructing the lung type,Zhike Pingchuan Shierwei mixture can significantly reduce levels of inflammatory factors in peripheral blood,such as IgE,TNF-αand IL-8,improve the lung function and cough-related quality of life,and relieve symptoms of cough.
