摘要
目的研究右美托咪定复合羟考酮超前镇痛对高血压脑出血患者的认知功能及肾功能的影响。方法选取2019年6月至2020年5月本院接收的高血压脑出血患者82例,按随机数字法分为两组,各41例。参照组采用羟考酮超前镇痛,研究组采用右美托咪定复合羟考酮超前镇痛。比较两组术前、术后24 h认知功能状况和肾功能情况以及术后各时段疼痛情况和不良反应发生情况。结果术前,两组MMSE、MoCA认知功能评分以及BUN、α1-MG和SCr等肾功能指标比较差异均无统计学意义;术后24 h,研究组MMSE、MoCA评分均高于参照组,研究组BUN、α1-MG和SCr水平均低于参照组,术后2、6、24 h,研究组疼痛评分均低于参照组,差异均有统计学意义(P<0.05)。两组不良反应发生率比较差异无统计学意义。结论对高血压脑出血病患采用右美托咪定复合羟考酮超前镇痛有利于改善术后认知功能及肾功能,缓解术后疼痛,并具有较高的安全性。
Objective To study the effect of dexmedetomidine combined with oxycodone on cognitive function and renal function in patients with hypertensive cerebral hemorrhage. Methods 82 patients with hypertensive intracerebral hemorrhage received from June 2019 to May 2020 were selected and they were randomly divided into two groups, with 43 cases in each group. The reference group was treated with oxycodone for preemptive analgesia, and study group was treated with dexmedetomidine combined with oxycodone for preemptive analgesia. The cognitive function,renal function before operation and 24 h after operation, pain and adverse reactions were compared between the two groups. Results There was no significant difference in the MMSE, MoCA cognitive function scores, BUN, α1-MG and SCr between the two groups. 24 hours after operation, the MMSE and MoCA scores of the study group were higher than those of the reference group;the BUN, α1-MG and SCr levels of the study group were lower than the reference group, at 2 h, 6 h and 24 h after operation, the pain scores were lower than those of the reference group, the difference was statistically significant(P<0.05). There was no statistically significant difference in the incidence of adverse reactions between the two groups.Conclusion Dexmedetomidine combined with oxycodone for preemptive analgesia in patients with hypertensive cerebral hemorrhage is beneficial to improve postoperative cognitive function and renal function, relieve postoperative pain, and has high safety.
