摘要
目的:探讨手法复位结合弹性髓内钉固定术对肱骨干骨折患儿手术指标的影响。方法:选取2017年4月~2019年5月本院肱骨干骨折患儿62例,按随机对照原则分为参照组和研究组。参照组实施手法复位结合石膏固定治疗,研究组实施手法复位结合弹性髓内钉固定术治疗,对比两组患儿手术及术后恢复情况,随访统计两组患儿肩肘关节活动度及肩肘关节功能评分,对比两组并发症发生率。结果:研究组肩肘关节活动度高于参照组,差异明显(P<0.05);研究组Mayo肘关节功能评分、Constant评分均高于参照组,差异明显(P<0.05);研究组术后7d、石膏拆除7d的上肢肿胀周径短于参照组,差异明显(P<0.05);研究组并发症发生率3.23%,低于参照组的25.81%,对比差异明显(P<0.05)。结论:手法复位联合弹性髓内钉固定可改善肱骨干骨折患儿术后恢复,提升患儿关节功能并降低并发症风险。
Objective:To investigate the effect of manual reduction combined with elastic intramullary nail fixation on surgical index in children with brachiohumeral fracture.Methods:62 children with humeral shaft fracture in our hospital from April 2017 to May 2019 were selected and divided into reference and study groups according to the principle of randomized control.According to the group implementing manual reduction combined with gypsum fixation treatment,the research group performed manual reduction combined with elastic intramedullary nail fixation.Comparing the surgery and postoperative recovery of the two groups,follow-up statistics of shoulder and elbow activity and shoulder and elbow function scores in the two groups,and compared the incidence of complications in the two groups.Results:The shoulder and elbow activity was higher than the reference group(P<0.05);Mayo elbow function score and Constant score were higher than the reference group(P<0.05);the upper limb swelling 7d and 7d after gypsum removed than the reference group(P<0.05);the study group was 3.23%,lower than 25.81%in the reference group,and significantly different(P<0.05).Conclusion:Manual reduction combined with elastic intramedullary nail fixation can improve the postoperative recovery of children with humeral shaft fracture,simultaneously improve their joint function and reduce the risk of complications.
