GnRH激动剂对围绝经期子宫肌瘤患者胰岛素敏感性的影响被引量：3

摘要目的探讨促性腺激素释放激素激动剂(GnRH)-α对围绝经期子宫肌瘤患者胰岛素敏感性和胰岛素抵抗相关因素的影响。方法随机选取35例诊断为子宫肌瘤的患者。在注射GnRH-α前和注射后6个月时,分别收集患者血清,检测空腹血糖、胰岛素、性激素、性激素结合蛋白(SHBG)、血管炎症标志物和细胞因子。结果所有患者胰岛素、空腹血糖水平和稳态模型评估胰岛素抵抗指数(HOMA-IR)均无显著性改变。在GnRH-α治疗前存在低HOMA-IR的患者,治疗后胰岛素、空腹血糖和HOMA-IR均出现显著升高,睾酮水平显著降低(P<0.05)。在GnRH-α治疗前存在高HOMA-IR的患者,治疗后胰岛素、HOMA-IR和SHBG水平显著降低,高敏C反应蛋白(hs-CRP)、可溶性细胞黏附分子(sICAM)-1、选择素-E和单核细胞趋化蛋白(MCP)-1均显著升高(P<0.05)。结论GnRH-α对围绝经期子宫肌瘤患者胰岛素敏感性的影响与治疗前患者自身胰岛素敏感性基础值密切相关。
