摘要
目的观察丘脑水平横断面超声评估孕11~13^(+6)周胎儿中脑导水管及其与头臀径的关系的价值。方法选取216名孕妇,并根据孕周将其分为3组:A组(n=50)孕11~11^(+6)周,B组(n=103)孕12~12^(+6)周,C组(n=63)孕13~13^(+6)周;并于胎儿丘脑水平横断面超声观察中脑导水管及与头臀径的关系。结果216胎中,204胎(A组45胎、B组99胎、C组60胎)中脑导水管显示良好,产前超声检查胎儿中枢神经系统均未见明显异常,且各组显示率差异无统计学意义(χ^(2)=2.507,P=0.286);12胎中脑导水管显示不清,其中10胎未见明显中枢神经系统异常,梗阻性脑积水、开放性脊柱裂合并脑积水各1胎,后均终止妊娠,胎儿尸体解剖检查结果与产前超声检查结果一致。214例出生后中枢神经系统相关检查均未见明显异常。相关性及回归分析显示胎儿中脑导水管左右径与头臀径呈线性负相关(r=-0.168,P<0.05)。结论于丘脑水平横断面超声观察孕11~13周^(+6)胎儿中脑导水管及与头臀径的关系可为预测胎儿中枢神经系统结构异常提供参考依据;胎儿中脑导水管显示不清提示存在中枢神经系统异常风险。
Objective To explore the value of transverse section of thalamus ultrasonic observation on midbrain aqueduct and the relationship of midbrain aqueduct with crown-rump length of fetus at 11—13^(+6) weeks.Methods Totally 216 pregnant women were enrolled and divided into 3 groups according to gestational weeks,including group A(n=50)11—11^(+6) weeks,group B(n=103)12—12^(+6) weeks and group C(n=63)13—13^(+6) weeks.The midbrain aqueduct and its relationship with crown-rump length were observed on transverse section of thalamus with ultrasound.Results Among 216 fetuses,204(including 45,99 and 60 fetuses respectively in group A,B and C)clearly showed midbrain aqueduct,and no obvious abnormality of central nervous system was found with prenatal ultrasound.There was no significant difference of display rate among group A,B and C(χ^(2)=2.507,P=0.286).The midbrain aqueduct was not clearly depicted in 12 fetuses,including 10 fetuses without obvious abnormality of central nervous system,1 fetus with obstructive hydrocephalus and the rest 1 with open spina bifida complicated with hydrocephalus.The pregnancies of the latter 2 were terminated,and the results of autopsy were consistent with those of prenatal ultrasound.No obvious abnormality was found during central nervous system examinations of 214 newborns.Correlation and regression analysis showed that there was a linear negative correlation between fetal crown-rump length and left-right diameter of midbrain aqueduct(r=-0.168,P<0.05).Conclusion Fetal midbrain aqueduct and its relationship with crown-rump length of fetus at 11—13^(+6) weeks could be observed on transverse section of thalamus with ultrasound,hence providing references for predicting structural abnormalities of fetal central nervous system.Midbrain aqueduct not able to be clear displayed indicated the risk of central nervous system disorders.
