摘要
目的探究彩超与超声造影联合应用对乳腺导管内乳头状瘤(IDP)的诊断意义。方法选取2017年2月至2019年2月本院收治的90例疑似IDP患者,分别采用彩色多普勒超声检查诊断和彩超联合超声造影检查诊断,所有患者均行手术治疗,以病理切片结果为金标准,分析彩超与超声造影联合对IDP的诊断价值。结果术后确诊80例为IDP患者,10例非IDP患者,以病理切片作为金标准,彩超诊断敏感度为80.00%,Kappa值为0.229;彩超联合超声造影诊断敏感度为96.25%,Kappa值为0.793。经彩超检查IDPⅠ型诊断符合率为100.00%,Ⅱ型诊断符合率为82.14%,Ⅲ型诊断符合率为50.00%,Ⅳ型诊断符合率为42.86%,Ⅴ型诊断符合率为60.00%,合计为81.25%;彩超联合超声造影IDPⅠ型诊断符合率为100.00%,Ⅱ型诊断符合率为96.43%,Ⅲ型诊断符合率为100.00%,Ⅳ型诊断符合率为85.71%,Ⅴ型诊断符合率为80.00%,合计为97.50%,彩超联合超声造影诊断IDP的诊断率较高。结论彩超与超声造影的联合诊断对IDP敏感度高,准确度高,利于疾病分型,值得临床应用。
Objective To explore the diagnostic significance of combined application of color Doppler ultrasound and contrast-enhanced ultrasound in the diagnosis of breast intraductal papilloma(IDP).Methods A total of 90 suspected IDP patients admitted to our hospital from February 2017 to February 2019 were selected and they were diagnosed by color Doppler ultrasound and color Doppler ultrasound combined with contrast-enhanced ultrasound.All patients were treated with surgery,and the results of pathological biopsy were treated as gold standard,analyze the diagnostic value of color Doppler ultrasound combined with contrast-enhanced ultrasound in IDP.Results 80 cases were diagnosed as IDP patients and 10 cases were non-IDP patients.Pathology was used as the gold standard.The diagnostic sensitivity of color Doppler ultrasound was 80.00%and the Kappa value was 0.229;the sensitivity of color Doppler ultrasound combined with contrast-enhanced ultrasound was 96.25%,and the Kappa value was 0.793.The diagnosis rate of IDP type I was 100.00%,the diagnosis rate of type II was 82.14%,the diagnosis rate of type III was 50.00%,the diagnosis rate of typeⅣwas 42.86%,the diagnosis rate of type V was 60.00%,and the total was 81.25%;the diagnostic rate of IDP type I by contrast-enhanced ultrasound is 100.00%,type II is 96.43%,type III is 100.00%,type IV is 85.71%,type V is 80.00%,and the total is 97.50%.Color Doppler ultrasound combined with ultrasound The diagnostic rate of IDP is relatively high.Conclusion The combined diagnosis of color Doppler ultrasound and contrast-enhanced ultrasound is highly sensitive and accurate to IDP,which is good for disease classification and it is worthy of clinical application.
