摘要
目的:分析肺炎支原体肺炎(MPP)合并右心室血栓患儿的超声心动图特点及其转归情况。方法:对我院2016年至2019年,心脏超声科诊断5例以MPP为首诊的右心室血栓住院患者的超声心动图资料进行回顾分析。结果:患者临床均诊断为重症支原体肺炎(SMPP),且均存在D-Dimer增高。所有患者心脏占位性病变全部经超声心动图首先检出。患者心脏房室内径及心脏功能在病程期间均未见异常。血栓位置均为位于右心室中部,大小为8.5~18.2 mm,与周围三尖瓣腱索及肌小梁关系紧密;呈中等偏强回声,类球形,边缘均不整齐,整体变性能力小。患者下肢血管超声均未见血栓形成。除1例手术取出血栓外,其余在随访期间均消失。结论:MPP合并心室血栓病例罕见,当患者存在凝血相关的血液指标异常时,应当及时行心脏超声检查。超声心动图能良好地显示占位性病变详细情况并能定期随访血栓变化。
Objective:To analyze the echocardiographic characteristics and outcome of mycoplasma pneumoniae pneumonia(MPP)with right ventricular thrombus.Methods:The echocardiographic data of 5 inpatients with right ventricular thrombus diagnosed by echocardiography from 2016 to 2019 were analyzed retrospectively.Results:All patients were diagnosed as severe mycoplasma pneumonia,and D-dimer increased.All patients’cardiac thrombuses were detected by echocardiography.There was no abnormality in atrioventricular diameter and cardiac function during the course of the disease.The thrombus was located in the middle of the right ventricle,the size was 8.5-18.2 mm,which was under the tricuspid valve closely related to the chorda.The thrombus was medium to strong echo,spherical like,with irregular edges and little overall denaturation ability.No thrombus was found in the lower extremity vessels.Except for one thrombus,the rest disappeared during the follow-up.Conclusions:the cases of MPP with VT are rare.If there is abnormal blood index related to coagulation,echocardiography should be performed in time.Echocardiography can well show the details of space occupying lesions and regularly follow up thrombus changes.
