摘要
目的探讨波长595 nm脉冲染料激光联合他扎罗汀乳膏和百癣夏塔热片对寻常痤疮患者皮脂分泌状况、血清白细胞介素-4(Interleukin-4,IL-4)、γ干扰素(Interferon-γ,IFN-γ)及血清睾酮(Testosterone,T)、雌二醇(Estradiol 2,E2)水平的影响。方法选择2018年3月至2020年6月,寻常痤疮患者120例,按照随机数字表法分为A组、B组和C组,每组患者各40例,其中A组仅采用脉冲染料激光治疗;B组采用脉冲染料激光联合他扎罗汀乳膏治疗;C组采用脉冲染料激光联合他扎罗汀乳膏和百癣夏塔热片联合治疗,三组激光治疗均为波长595 nm,脉宽1.4~4.5 ms,能量密度为7~8 J/cm^(2),共治疗4次,每次治疗间隔3周,共治疗9周。末次治疗后3周观察各组临床疗效、治疗前后的红斑、色素沉着、皮肤病生活质量指数(Dermatology lifequality index,DLQI)评分、皮脂分泌状况、血清T、E2、IL-4、IFN-γ水平及治疗期间不良反应。结果末次治疗后3周,C组总有效率(95.0%)显著高于B组(80.0%)和A组(55.0%),B组总有效率显著高于A组,三组间总有效率比较差异具有统计学意义(P<0.05)。末次治疗后3周各组患者红斑、色素沉着、DLQI评分、皮脂分泌率、血清T、IL-4均较治疗前显著降低(P<0.05);血清E2、IFN-γ、IFN-γ/IL-4较治疗前显著升高(P<0.05);C组红斑、色素沉着、DLQI评分、皮脂分泌率、血清T、IL-4显著低于B组和A组(P<0.05),C组E2、IFN-γ、IFN-γ/IL-4显著高于B组和A组(P<0.05);B组红斑、色素沉着、DLQI评分、皮脂分泌率、血清T、IL-4显著低于A组,B组E2、IFN-γ、IFN-γ/IL-4显著高于A组(P<0.05)。三组不良反应发生率比较无统计学意义(P>0.05)。结论595 nm脉冲染料激光联合他扎罗汀乳膏和百癣夏塔热片治疗寻常痤疮具有较好的临床效果。
Objective To investigate the effect of 595 nm pulsed dye laser combined with tazarotene cream and Baixuan Xiatare Pian on sebum secretion,serum interleukin-4(IL-4),interferon-γ(IFN-γ)and serum testosterone(T)and estradiol 2(E2)levels in patients with acne vulgaris.Methods Totally 120 patients with acne vulgaris treated during the period from Mar.2018 to Jun.2020 were selected and divided randomly into 3 groups:Group A,B and C,40 cases in each group.The patients in Group A were treated with 595 nm pulsed dye laser,those in Group B with 595 nm pulsed dye laser and tazarotene cream,and those in Group C with 595 nm pulsed dye laser plus tazarotene cream and Baixuan Xiatare Tablet.As to the laser treatment,the wavelength was 595 nm,the pulse width 1.4-4.5 ms and the energy density 7-8 J/cm^(2),a total of 4 times,each treatment interval of 3 weeks,a total of 9 weeks of treatment.Clinical efficacy,erythema,pigmentation,dermatology life quality index(DLQI)score,sebum secretion,serum T,E2,IL-4,IFN-γlevels with three weeks after the last treatment,and adverse reactions during the treatment.Results Three weeks after the last treatment,the total effective rate(95.0%)of Group C was significantly higher than that(Group B:80.0%,Group A:55.0%)of group B and group A,and the total effective rate of Group B was remarkably higher than that of Group A.The differences among the three groups in the total effective rate were statistically significant(P<0.05).Three weeks after the last treatment,the erythema,pigmentation,DLQI score,sebum secretion rate,serum T and IL-4 in each group were significantly lower than those before treatment(P<0.05),and the serum E2,IFN-γ,IFN-γ/IL-4 significantly higher than those before treatment(P<0.05).The erythema,pigmentation,DLQI score,sebum secretion rate,serum T and IL-4 in Group C were significantly lower than those in Group B and Group A(P<0.05),but the E2,IFN-γ,and IFN-γ/IL-4 significantly higher than those in Group B and Group A(P<0.05).The erythema,pigmentation,DLQI score,sebum secretion rate,serum T and IL-4 in Group B were significantly lower than those in Group A,and the E2,IFN-γ,IFN-γ/IL-4 significantly higher than those in Group A(P<0.05).There was no significant difference in the incidence of adverse reactions among the three groups(P>0.05).Conclusions The combination of 595 nm pulsed dye laser with tazarotene cream and Baixuan Xiatare Tablet has a good effect on acne vulgaris.
