摘要
目的比较彩色多普勒超声引导下穿刺抽吸冲洗和切开引流治疗哺乳期乳腺脓肿的效果。方法选取我院2019年11月至2020年8月收治的68例哺乳期乳腺脓肿患者为研究对象,根据抽签法将其分为对照组和观察组,每组34例。对照组采取传统切开引流治疗,观察组采取彩色多普勒超声引导下穿刺抽吸冲洗治疗。比较两组的康复情况、治疗效果、并发症发生情况、随访情况、生活质量评分及治疗满意度。结果观察组的VAS评分低于对照组,伤口脓腔愈合时间、瘢痕长度短于对照组,平均换药次数少于对照组(P<0.05)。观察组的治愈率高于对照组,并发症总发生率低于对照组(P<0.05)。观察组的术后复发率低于对照组,术后继续哺乳率高于对照组(P<0.05)。观察组的GQOLI-74各项评分、治疗满意度高于对照组,差异具有统计学意义(P<0.05)。结论彩色多普勒超声引导下穿刺抽吸冲洗治疗哺乳期乳腺脓肿的效果优于切开引流治疗,可降低患者的疼痛程度、复发率、并发症发生率,提升继续哺乳率、生活质量及治疗满意度。
Objective To compare the effects of puncture,aspiration and irrigation guided by color Doppler ultrasound and incision and drainage in the treatment of breast abscess during lactation.Methods Sixty-eight patients with breast abscess during lactation treated in our hospital from November 2019 to August 2020 were selected as the research objects and divided into control group and observation group by drawing lots method,with 34 cases in each group.The control group was treated with traditional incision and drainage,and the observation group was treated with puncture,aspiration and irrigation guided by color Doppler ultrasound.The rehabilitation,treatment effect,complications,follow-up,quality of life score and treatment satisfaction were compared between the two groups.Results The VAS score of the observation group was lower than that of the control group,the wound purulent cavity healing time and scar length were shorter than those of the control group,and the average dressing change times was less than that of the control group(P<0.05).The cure rate of the observation group was higher than that of the control group,and the total incidence of complications was lower than that of the control group(P<0.05).The postoperative recurrence rate of the observation group was lower than that of the control group,while the continued lactation rate after operation was higher than that of the control group(P<0.05).The GQOLI-74 scores and treatment satisfaction of the observation group were higher than those of the control group,and the differences were statistically significant(P<0.05).Conclusion The puncture,aspiration and irrigation guided by color Doppler ultrasound in the treatment of breast abscess during lactation has a better effect compared with incision and drainage.It can reduce the degree of pain,recurrence rate and incidence of complications,and improve the continuous lactation rate,quality of life and treatment satisfaction.
