摘要
目的分析多层螺旋CT尿路造影(CTU)对肾结核的诊断价值。方法回顾性分析2016年1月至2020年6月78例泌尿系感染患者的CTU成像资料。以手术病理结果诊断肾结核为金标准,采用受试者工作特征曲线(ROC)评估CTU成像在肾结核疾病中的诊断价值。采用Kappa值评价CTU成像诊断结果与病理结果的一致性。结果78例患者中共有42例肾结核。肾结核组患者的CTU特征主要表现为肾体积缩小、肾脓肿、肾盏或肾盂壁增厚、输尿管壁增厚、膀胱壁增厚、肾实质钙化、肾结石、肾积水等。CTU成像鉴别诊断肾结核的诊断价值高达97%,敏感度、特异度分别93%,89%,鉴别诊断结果与病理结果具有较好的一致性(Kappa值为0.793),差异有统计学意义(P<0.01)。结论CTU检查对肾结核疾病的诊断价值较高,可以在泌尿系疾患中推广使用。
Objective To analyze the diagnostic value of multi-slice spiral computed tomography urography(CTU)in renal tuberculosis.Methods The surgical pathology results were used as the significant standand,78 patients with urinary tract infection were retrospectively analyzed between Januarary 2016 and June 2020.The receiver operator characteristic curve(ROC)was used to evaluate the diagnostic value of CTU imaging in renal tuberculosis.Kappa value was used to evaluate the consistency of CTU imaging and pathological results.Results There were 42 cases of renal tuberculosis in 78 patients.In the renal tuberculosis group,the main CTU features were renal volume reduction,renal abscess,renal calyceal or pelvic wall thickening,ureteral wall thickening,bladder wall thickening,renal parenchymal calcification,renal calculi,hydronephrosis,etc.The diagnostic value of CTU imaging in differential diagnosis of renal tuberculosis was 97%,the sensitivity and specificity were 93%and 89%.The differential diagnosis results were consistent with the pathological results(Kappa value was 0.793),and the difference was statistically significant(P<0.01).Conclusion CTU examination has high diagnostic value for renal tuberculosis and can be widely used in urinary diseases.
