摘要
目的探究子宫颈上皮内瘤变(CIN)患者子宫颈环形电切除术后妊娠产妇分娩方式与妊娠结局及其影响因素Logistic回归分析。方法于2017年8月至2020年8月抽选在我院进行子宫颈环形电切除术治疗后的52例CIN妊娠患者作为研究组,同期选择在我院分娩的40例健康孕妇作为对比组,对比两组的分娩方式和妊娠结局。根据妊娠结局将研究组分为妊娠结局差组、妊娠结局佳组,并分析影响CIN妊娠患者妊娠结局的因素。结果研究组的剖宫产率高于对比组,而阴道分娩率低于对比组(P<0.05)。研究组的早产、产后出血、胎膜早破发生率均高于对比组(P<0.05)。经Logistic因素分析,剩余宫颈管长度、切除组织体积是影响CIN妊娠患者妊娠结局的独立危险因素(P<0.05)。结论行子宫颈环形电切除术治疗的CIN妊娠患者其剖宫产、早产、产后出血、胎膜早破发生风险高,而且剩余宫颈管长度、切除组织体积是影响CIN妊娠患者妊娠结局危险因素。
Objective To explore the relationship between delivery mode and pregnancy outcome of pregnant women with cervical intraepithelial neoplasia(CIN)after loop electrosurgical excision of cervix and logistic regression analysis of its influencing factors.Methods From August 2017 to August 2020,52 cases of CIN pregnancy patients who received cervical loop electrosurgical excision in our hospital were selected as the study group,and 40 cases of healthy pregnant women who delivered in our hospital were selected as the control group at the same time.The delivery mode and pregnancy outcome of the two groups were compared.According to the pregnancy outcome,the study group was divided into poor pregnancy outcome group and good pregnancy outcome group,and the influencing factors of pregnancy outcome were analyzed.Results The cesarean section rate of the study group was higher than that of the control group,while the vaginal delivery rate was lower than that of the control group(P<0.05).The incidence of premature birth,postpartum hemorrhage and premature rupture of membranes in the study group were higher than those in the control group(P<0.05).Logistic analysis showed that the length of residual cervical canal and the volume of excised tissue were the independent risk factors of pregnancy outcome in patients with CIN(P<0.05).Conclusion The risk of cesarean section,premature delivery,postpartum hemorrhage and premature rupture of membranes is high in patients with CIN who underwent loop electrosurgical excision of cervix.The length of residual cervical canal and the volume of excised tissue are the risk factors of pregnancy outcome in patients with CIN.
