超声引导下喉上神经阻滞联合羟考酮在支气管镜检查术患者的应用 被引量:4

摘要 目的探讨超声引导下喉上神经阻滞联合静脉注射羟考酮在支气管镜检查手术患者的应用效果。方法行支气管镜检查患者96例随机分为两组,每组48例。两组均行5 mL 2%利多卡因喷喉和3 mL 2%利多卡因雾化吸入表面麻醉。A组静脉注射丙泊酚1~2 mg/kg和雷米芬太尼0.05~0.10μg/kg;B组静脉注射羟考酮0.1 mg/kg,并采用1%利多卡因3 mL行超声引导下双侧喉上神经阻滞。记录麻醉前(T0)、麻醉后(T1)和气管镜插入声门时(T2)的BP、HR和SpO_(2),观察术后苏醒和不良反应。结果与A组比较,B组术中基本处于清醒合作状态,血压、心率波动较小,低氧血症发生率低(0 vs.20.8%),术后不良反应发生率低(18.8%vs.79.2%)(P<0.05)。结论对于行支气管镜检查术患者,超声引导下行1%利多卡因喉上神经阻滞联合静脉注射羟考酮能有效减轻手术刺激,提高检查安全性,减少不良反应。 Objective To observe the efficacy of ultrasound-guided superior laryngeal nerve block combined with oxycodone in the patients undergoing bronchoscopy.Methods A total of 96 patients undergoing bronchoscopy were randomly divided into two groups with 48 cases each.Laryngeal spray with 2%lidocaine 5 mL and airway atomization inhalation with 2%lidocaine 3 mL were performed in both groups.Ultrasound-guided superior laryngeal nerve block of both sides with 1% lidocaine 3 mL was performed and oxycodone 0.1 mg/kg was intravenously injected in group B.Propofol 1~2 mg/kg and remifentanil 0.05~0.10 μg/kg were intravenously injected in group A.BP,HR and SpO_(2) were recorded at the time points of before anesthesia(T0),after anesthesia(T1) and during bronchoscope insertion into the acoustic valve(T2).Postoperative awakening and adverse reactions were observed.Results Compared with group A,the patients in group B were basically in a state of awake and cooperation, with small fluctuation of blood pressure and heart rate, and lower incidence rates of hypoxemia during surgery(0 vs.20.8%) and postoperative adverse responses(18.8% vs.79.2%)(P<0.05).Conclusion Combined use of ultrasound-guided 2% lidocaine superior laryngeal nerve block and intravenously injection of oxycodone 0.1 mg/kg can effectively reduce surgical stimulation, improve examination safety, and reduce adverse reactions in the patients undergoing bronchoscopy.
