摘要
目的探讨胫腓联合损伤应用彩色多普勒超声的诊断效果。方法选取2019年6月至2020年5月新余市人民医院收治的疑似胫腓联合损伤患者58例作为研究对象,术中常规对患者进行胫腓联合探查,以此作为金标准,术前均进行正侧位X线和彩色多普勒超声检查,由不同影像学医生对胫腓联合损伤情况进行评价,再将诊断结果与手术探查结果进行统计学分析,评价诊断效能。结果58例疑似胫腓联合损伤患者经过手术探查,共有40例存在胫腓联合损伤,18例不存在损伤。X线诊断显示,40例胫腓联合损伤患者阳性23例,阴性17例;18例无损伤者阳性8例,阴性10例。超声诊断显示,40例胫腓联合损伤患者阳性38例,阴性2例;18例无损伤者阳性1例,阴性17例。彩色多普勒超声诊断敏感度为95.00%、特异度为94.44%、阳性预测值为97.44%、阴性预测值为89.47%,均高于X线诊断,差异有统计学意义(P<0.05)。结论在胫腓联合损伤诊断中应用彩色多普勒超声具有较高的准确性和可靠性,可有效提高诊断效果,值得临床推广应用。
Objective To investigate the diagnostic effect of color Doppler ultrasonography in tibiofibular syndesmosis injury.Methods A total of 58 patients with suspected tibiofibular syndesmosis injury admitted to Xinyu People's Hospital from June 2019 to May 2020 were selected as the study subjects.During the operation,tibiofibular syndesmosis exploration was routinely performed on the patients as the gold standard.Anteroposterior and lateral X-rays and color Doppler ultrasonography were performed.Different imaging doctors evaluated the tibiofibular syndesmosis injury,and then statistically analyzed the diagnostic results and surgical exploration results to evaluate the diagnostic efficacy.Results 58 cases of suspected tibiofibular syndesmosis injury underwent surgical exploration.Forty cases had tibiofibular syndesmosis injury,and 18 cases had no injury.X-ray diagnosis showed that 23 of 40 patients with tibiofibular syndesmosis injury were positive,and 17 were negative;8 of 18 patients without injury were positive,and ten were negative.Ultrasonographic diagnosis showed that 38 of 40 patients with tibiofibular syndesmosis injury were positive,and two were negative;1 of 18 patients without injury was positive,and 17 were negative.The sensitivity,specificity,positive predictive value,and negative predictive value of color Doppler ultrasonography were 95.00%,94.44%,97.44%,and 89.47%,respectively,which were significantly higher than those of X-ray diagnosis,the difference was statistically significant(P<0.05).Conclusion The application of color Doppler ultrasonography in the diagnosis of tibiofibular syndesmosis injury has high accuracy and reliability and can effectively improve the diagnostic effect.It is worthy of being widely popularized in clinical application.
