摘要
目的探讨集束化干预措施在全身麻醉术后患者苏醒期尿管源性躁动预防中的应用。方法选取2020年8月至2021年2月江西省九江市中医医院麻醉科进行全身麻醉的500例腹部手术患者作为研究对象,采用随机数字表法将其分为对照组与干预组,每组各250例。对照组患者采用常规手术室护理模式,干预组患者采用集束化干预措施。比较两组患者的视觉模拟评分法(VAS)评分、Ramsay镇静评分、尿管刺激发生率和躁动发生率。结果干预后,干预组患者的VAS评分低于对照组,差异有统计意义(P<0.05);干预后,干预组患者的Ramsay镇静评分高于对照组,差异有统计意义(P<0.05);干预组患者的尿管刺激发生率(18.8%)、躁动发生率(6.8%)低于对照组(42.8%、29.6%),差异有统计学意义(P<0.05)。结论集束化干预措施可以降低全身麻醉术后患者苏醒期躁动发生率、尿管刺激发生率,缓解患者术后的疼痛,对患者全身麻醉术后患苏醒期尿管源性躁动具有一定的预防作用。
Objective To investigate the application of cluster intervention in the prevention of urogenic agitation during recovery period after general anesthesia.Methods A total of 500 patients undergoing abdominal surgery under general anesthesia from August 2020 to February 2021 in Department of Anesthesiology,Jiujiang Hospital of Traditional Chinese Medicine in Jiangxi Province were selected as the research subjects.The patients were divided into the control group and the intervention group by random number table method,with 250 cases in each group.The control group adopted routine operation room nursing mode.In the intervention group,cluster intervention measures were implemented.Visual analogue scale(VAS)score,Ramsay sedation score,incidence of urinary duct irritation and agitation were compared between the two groups.Results After intervention,VAS score in the intervention group was lower than that in the control group,the difference was statistically significant(P<0.05).After intervention,Ramsay sedation score in the intervention group was higher than that in the control group,the difference was statistically significant(P<0.05).The incidence of urinary duct irritation(18.8%)and agitation(6.8%)in the intervention group were lower than those in the control group(42.8%and 29.6%),the differences were statistically significant(P<0.05).Conclusion Cluster intervention can reduce the occurrence of restlessness and urinary duct irritation,relieve postoperative pain and drowsiness,and has a certain preventive effect on the occurrence of urinary duct restlessness after general anesthesia.
