摘要
目的探讨链霉蛋白酶颗粒联合二甲硅油散、达克罗宁胶浆用于胃镜检查术前准备中的有效性。方法收治的行胃镜检查的114例出院患者,根据患者胃镜检查前使用药物不同分为对照组(n=59)和研究组(n=55)。对照组患者胃镜检查前使用二甲硅油散联合达克罗宁胶浆,研究组患者使用链霉蛋白酶颗粒联合二甲硅油散、达克罗宁胶浆。通过查阅电子病历,对2组患者食管、胃底、胃体、胃窦及十二指肠黏膜清晰度进行评分,并比较2组患者微小病灶检出及不良反应发生情况。结果研究组食管、胃底、胃体、胃窦及十二指肠黏膜清晰度明显高于对照组,差异均有统计学意义(P<0.05);研究组微小病变检出率显著高于对照组,差异有统计学意义(P<0.05);对2组微小病变进行病理结果分析发现,研究组显著提高了胃黏膜组织内炎性细胞的检出率(P<0.05)。此外,2组患者均未发生药物相关的不良反应和并发症。结论链霉蛋白酶颗粒联合二甲硅油散、达克罗宁胶浆能有效改善胃镜检查中视野的清晰度,有利于胃黏膜微小病变的发现,且不良反应少,值得临床推广。
Objective To investigate the effects of premedication of pronase combined with dimethylpolysiloxane and dyclonine gel before upper gastrointestinal endoscopy.Methods A total of 114 patients who underwent gastroscopy from October to December 2018 and October to December 2019 were randomly divided into control group(n=59)and research group(n=55).The patients in control group were treated by dimethylpolysiloxane and dyclonine gel before gastroscopy,however,the patients in research group were treated by pronase,dimethylpolysiloxane and dyclonine gel.By referring to electronic medical records,the gastric mucosal visibility was graded according to the esophagus,gastric fundus,gastric body,gastric antrum and duodenal in both groups.In addition,the detection rate of microscopic lesions and incidences of adverse drug reactions were observed and compared between two groups.Results The mucosa visibilities of esophagus,gastric fundus,gastric body,gastric antrum and duodenal in research group were significantly higher than those in control group(P<0.05).The detection rate of microscopic lesions in research group was significantly higher than that in control group(P<0.05).The pathological analysis of small lesions showed that the detection rate of inflammatory cells in gastric mucosal tissue was significantly increased in research group,however,the detection rate of intestinal metaplasia was similar between the two groups.Moreover no drug-related adverse reactions or complications were found in both groups.Conclusion Application of premedication of pronase combined with dimethylpolysiloxane and dyclonine gel for endoscopy can improve gastric mucosal visibility and contribute to the detection of microscopic lesions,with less adverse reactions,which is worthy of clinical promotion.
