摘要
目的:探讨彩色多普勒超声在子宫肌瘤诊断中的应用价值。方法:回顾性分析本院2019年10月~2020年9月收治的经病理确诊的子宫肌瘤患者78例临床资料,患者分别进行腹部超声、经阴道超声检查,并以病理检查结果为金标准,比较腹部超声与经阴道超声对子宫肌瘤的诊断准确率、漏诊率及误诊率,比较肌瘤病变内部及周围血流状况。结果:腹部超声检查对子宫肌瘤的诊断准确率为85.90%、漏诊率为5.13%、误诊率为8.97%;经阴道超声检查对子宫肌瘤的诊断准确率为96.15%、漏诊率为2.56%、误诊率为1.28%。两种检查方法对子宫肌瘤的诊断准确率均较高,但经阴道超声的诊断准确率高于腹部超声结果,误诊率低于腹部超声结果(P<0.05);经阴道彩色多普勒超声检查结果显示,肌瘤周边组织Vd(5.23±0.77)cm/s、Vs(72.14±15.46)cm/s、RI(0.70±0.15)均高于肌瘤内部Vd(4.35±0.62)cm/s、Vs(58.82±10.58)cm/s、RI(0.56±0.12),P<0.05。结论:彩色多普勒超声对子宫肌瘤的诊断准确率高,临床通过结合子宫肌瘤内部及周围组织血流动力学变化,可进一步为子宫肌瘤的诊断提供参考依据,从而提高诊断率。
Objective:To explore the application value of color Doppler ultrasound in the diagnosis of uterine fibroids.Methods:The clinical data of 78 patients with pathologically diagnosed uterine fibroids who were admitted to our hospital from October 2019 to September 2020 were retrospectively analyzed.The patients underwent abdominal ultrasonography and transvaginal ultrasonography respectively,and the results of pathological examination were used as the gold standard to compare the diagnostic accuracy,missed diagnosis and misdiagnosis rate of abdominal ultrasonography and transvaginal ultrasonography for uterine fibroids.Comparison of blood flow in and around fibroids.Results:The diagnostic accuracy rate of abdominal ultrasonography for uterine fibroids was(85.90%),the missed diagnosis rate(5.13%),and the misdiagnosis rate(8.97%);the diagnostic accuracy rate of transvaginal ultrasonography for uterine fibroids was(96.15%),the missed diagnosis rate(2.56%),and the misdiagnosis rate(1.28%).The diagnostic accuracy of the two methods for uterine fibroids was higher,but the diagnostic accuracy of transvaginal ultrasonography was higher than that of abdominal ultrasonography,and the misdiagnosis rate was lower than that of abdominal ultrasonography(P<0.05);The results of transvaginal color Doppler ultrasonography showed that the Vd(5.23±0.77)cm/s,Vs(72.14±15.46)cm/s and RI(0.70±0.15)of the surrounding tissue were higher than those in the interior of leiomyoma Vd(4.35±0.62)cm/s,Vs(58.82±10.58)cm/s,RI(0.56±0.12),P<0.05.Conclusion:Color Doppler ultrasound has a high diagnostic accuracy for uterine fibroids,which can further provide a reference for the diagnosis of uterine fibroids by combining the hemodynamic changes of the tissue inside and around uterine fibroids,so as to provide the diagnostic rate.
