摘要
目的探讨对重症急性胰腺炎患者采用通腑安涤汤内服与灌肠治疗的临床疗效及对改善T细胞亚群的作用。方法选择龙口市人民医院2019年1月—2021年6月收治重症急性胰腺炎患者80例,按照随机数表法分为2组,各40例。对照组采用西医常规治疗,试验组则在西医治疗基础上联合通腑安涤汤内服与灌肠治疗,比较2组临床疗效,检测2组治疗前后血清淀粉酶(AMS)、脂肪酶(LPS)水平评估疾病严重程度,并检测T细胞亚群(CD3^(+)、CD4^(+)、CD4^(+)/CD8^(+))水平评估患者免疫功能。结果治疗后试验组治疗总有效率为95.00%(38/40)高于对照组的80.00%(32/40),差异有统计学意义(P<0.05);AMS、LPS水平均小于对照组,差异有统计学意义(P<0.05);CD3^(+)、CD4^(+)、CD4^(+)/CD8^(+)水平大于对照组,差异有统计学意义(P<0.05)。结论对重症急性胰腺炎患者采用通腑安涤汤内服与灌肠治疗干预可提高疾病治疗效果,能够减轻疾病严重程度,有利于改善机体免疫功能,值得推广。
Objective To investigate the clinical efficacy oral administration and enema of Tongfu Andi decoction in the treatment of severe acute pancreatiti and its effect on improving T cell subsets.Methods Eighty patients with severe acute pancreatitis admitted to Longkou People’s Hospital from January 2019 to June 2021 were divided into two groups according to the random number table method.The control group(n=40)received conventional western medicine treatment,and the experimental group(n=40)received oral administration of Tongfu Andi decoction and enema treatment on the basis of western medicine treatment.Clinical efficacy of the two groups was compared.Serum amylase(AMS)and lipase(LPS)levels before and after treatment were measured to assess the severity of the disease,and T cell subsets(CD3^(+),CD4^(+),CD4^(+)/CD8^(+))were measured to assess the immune function of patients.Results After treatment,the total effective rate of experimental group was 95.00%(38/40)higher than that of control group 80.00%(32/40),and the difference was statistically significant(P<0.05).The AMS and LPS levels were significantly lower than those of the control group(P<0.05).The levels of CD3^(+),CD4^(+) and CD4^(+)/CD8^(+) were higher than those of the control group.The difference was statistically significant(P<0.05).Conclusion The use of oral administration and enema of Tongfu Andi decoction intervention in patients with severe acute pancreatitis can improve the therapeutic effect of the disease,reduce the severity of the disease,and improve the immune function of the body,so it is worth promoting.
