Cynergy双波长脉冲染料激光治疗鲜红斑痣的对照研究

摘要 目的比较采用脉冲染料激光(pdsed dye laser,PDL)和Cynergy双波长脉冲染料激光治疗成年鲜红斑痣患者的疗效和安全性。方法收集2010年1月~2020年10月就诊于石家庄市人民医院皮肤科的61例鲜红斑痣患者资料。采用双波长脉冲染料治疗的31例患者为双波长组,采用585 nm脉冲染料激光治疗的30例为PDL组。比较两组间总体疗效、不良反应,并对两组间不同类型鲜红斑痣治疗效果进行比较。结果双波长组总有效率74.19%(23/31),高于PDL组的有效率40.00%(12/30),组间差异有显著统计学意义(χ^(2)=7.288,P=0.007)。双波长组IV型、V型、VI型鲜红斑痣治疗有效率均高于PDL组,其中VI型两组间差异有统计学意义(P<0.05)。PDL组和双波长组不良反应发生率分别为10.0%(3/30)、16.12%(5/31),两组比较,差异无统计学意义(χ^(2)=0.502,P=0.478)。结论对于成年鲜红斑痣患者,采用585 nm和1064 nm双波长激光治疗,疗效优于585 nmPDL,且不良反应可控,针对每例患者确定最佳剂量,可进一步降低不良反应。 Objective To compare the efficacy and safety of pulsed dye laser(PDL)and Cynergy dual wavelength pulsed dye laser in the treatment of adult port wine stain.Methods 61 patients with fresh erythema nevus were collected.31 patients treated with dual wavelength pulsed dye were in the dual wavelength group,and 30 patients treated with 585nm pulsed dye laser were in the PDL group.The overall efficacy and adverse reactions between the two groups were compared,and the therapeutic effects of different types of port wine stain between the two groups were compared.Results The total effective rate of dual wavelength group was 74.19%(23/31),which was higher than that of PDL group 40.00%(12/30).There was significant difference between the groups,χ^(2)=7.288,P=0.007.The effective rates of type IV,V and VI port wine stains in dual wavelength group were higher than those in PDL group,and there was significant difference between type VI and PDL groups(P<0.05).The incidence of adverse reactions in PDL group and dual wavelength group were 10.0%(3/30)and 16.12%(5/31)respectively.There was no significant difference between the two groups,χ^(2)=0.502,P=0.478.Conclusion For adult patients with port wine stain,the efficacy of 585 nm and 1064 nm dual wavelength laser treatment is better than 585 nm PDL,and the adverse reactions are controllable.Determining the optimal dose for each patient can further reduce the adverse reactions.
