基于PI-RADS 2.1探讨1.5T MRI两种不同b值FOV的DWI对前列腺病变的诊断价值 被引量:1

摘要 目的:以PI-RADS 2.1为基础,比较1.5T磁共振(MRI)两种b值、不同视野(FOV)的DWI对前列腺病变的诊断价值。方法:运用1.5T磁共振前瞻性搜集上海市徐汇区大华医院2019年12月—2020年12月收治的55例前列腺病变患者共60个前列腺病灶进行检查,其中弥散加权成像(DWI)分别采用b值1000 s/mm^(2)(A组)、b值1400 s/mm^(2)(B组)各自3种视野分别为PI-RADS 2.1版本推荐范围内的22 cm×22 cm(A1、B1组)及超出推荐范围的26 cm×26 cm(A2、B2组)、30 cm×30 cm(A3、B3组),对6组DWI图像的影像质量进行评价比较,所有患者均知情同意;并以病理结果为金标准统计各组DWI基于PI-RADS 2.1评分的诊断效能并进行比较。结果:经穿刺活检或手术证实其中28个为前列腺癌灶,32个为增生结节;图像质量评分比较:B2组>A2组>B3组>A3组>A1组>B1组(P<0.05);AUC:B2组(0.989)>A2组(0.954)>B3组(0.886)>A3组(0.816)>A1组(0.592)>B1组(0.558),差异均有统计学意义(P<0.05);灵敏度:B2组>A2组(P<0.05)>B3组>A3组>A1组、B1组(P<0.05);特异度:B2、A2组>B3组(P<0.05)>A3组>A1组、B1组。结论:以PI-RADS 2.1评分标准为基础,运用1.5 T MRI检查前列腺时,b值1400 s/mm^(2)、FOV为26 cm×26 cm的DWI影像质量更佳,诊断前列腺癌病变时更具价值。 Objective To compare the diagnostic value of diffusion-weighted imaging(DWI)with two b-values and different fields of view(FOV)using 1.5 T magnetic resonance imaging(MRI)in prostatic lesions based on PI-RADS 2.1.Methods A total of 60 prostate lesions in 55 patients with prostate lesions treated in Dahua Hospital of Xuhui District,Shanghai from December 2019 to December 2020 were prospectively collected by 1.5T MRI.DWI was carried out with b-value of 1,000s/mm^(2)(group A)and 1,400s/mm^(2)(group B),respectively.Further,3 FOV were adopted,22 cm×22 cm(group A1 and B1)within the recommended range of PI-RADS 2.1,and 26 cm×26 cm(group A2 and B2)and 30 cm×30 cm(group A3 and B3)exceeding the recommended range of PI-RADS 2.1.The quality of DWI images in the 6 groups was evaluated and compared.All the patients signed the informed consent.Additionally,the diagnostic efficiency of DWI based on PI-RADS 2.1 score in each group was calculated and compared with pathological results as the gold standard.Results Among the lesions,28 were confirmed as prostate cancer foci and 32 as hyperplastic nodules by needle biopsy or surgery.The comparison in image quality score showed that the group B2>group A2>group B3>group A3>group A1>group B1.AUC in the group B2(0.989)>the group A2(0.954)>the group B3(0.886)>the group A3(0.816)>the group A1(0.592)>the group B1(0.558),the differences were statistically significant(P<0.05).Sensitivity in the group B2>the group A2(P<0.05)>the group B3(P=0.054)>the group A3(P=0.191)>the group A1 and the group B1(P<0.05).Specificity in the group B2 and A2>the group B3(P<0.05)>the group A3(P=0.279)>the group A1 and B1(P=0.617).Conclusion Based on the scoring criteria of PI-RADS2.1,when examining the prostate using 1.5T MRI,the quality of DWI images is higher with b-value of 1,400s/mm^(2) and FOV of 26cm×26cm,and it is more valuable in the diagnosis of prostate cancer lesions.
