多频振动排痰联合密闭式吸痰法在ICU肺部感染机械通气患者中的应用效果研究 被引量:6

摘要 目的观察多频振动排痰联合密闭式吸痰法在ICU肺部感染机械通气患者中的应用效果。方法以2017年1月—2020年6月浦北县中医医院ICU收治的60例肺部感染机械通气患者为研究对象,随机平均分为对照组及研究组,每组30例。对照组采取常规叩背排痰法联合开放式吸痰法,研究组则采取多频振动排痰联合密闭式吸痰法,对比分析2组患者的ICU停留时间、机械通气时间、血氧饱和度及血气分析指标、排痰效果。结果研究组ICU停留时间、机械通气时间显著低于对照组(P<0.05)。2组患者治疗前血氧饱和度、PaO_(2)以及PaCO_(2)水平均无显著差异(P>0.05);治疗后2组血氧饱和度及PaO_(2)水平均升高,PaCO_(2)水平均降低,研究组显著优于对照组(P<0.05)。研究组的血痰发生率及气管黏膜损伤发生率显著低于对照组(P<0.05);研究组排痰有效率为96.67%,显著高于对照组的76.67%(P<0.05)。结论多频振动排痰联合密闭式吸痰法可以改善ICU肺部感染机械通气患者的血氧饱和度和血气分析指标,提升排痰效果,降低血痰和气管黏膜损伤发生率,缩短ICU停留时间和机械通气时间,有利于预后改善。 Objective To observe the effect of multi frequency vibration expectoration combined with closed sputum suction in ICU patients with pulmonary infection and mechanical ventilation.Methods 60 patients with pulmonary infection and mechanical ventilation treated in ICU of our hospital from January 2017 to June 2020 were randomly divided into control group and study group,with 30 cases in each group.The control group was treated with conventional buckled back sputum drainage combined with open sputum suction.The study group adopted multi frequency vibration sputum drainage combined with closed sputum suction.The ICU stay time,mechanical ventilation time,changes of blood oxygen saturation and blood gas analysis indexes before and after treatment,and sputum excretion effect were compared and analyzed between the two groups.Results The ICU stay time and mechanical ventilation time in the study group were significantly lower than those in the control group(P<0.05).There was no significant difference in blood oxygen saturation,PaO_(2) and PaCO_(2) between the two groups before treatment(P>0.05).After treatment,the levels of blood oxygen saturation and PaO_(2) increased significantly,and the level of PaCO_(2) decreased significantly in the two groups,and the study group was significantly better than the control group(P<0.05).The incidence of blood sputum and tracheal mucosal injury in the study group were significantly lower than those in the control group(P<0.05).The effective rate of sputum excretion in the study group was 96.67%,which was significantly higher than 76.67%in the control group(P<0.05).Conclusion Multi frequency vibration expectoration combined with closed sputum suction can improve the blood oxygen saturation and blood gas analysis indexes of patients with pulmonary infection in ICU,improve the expectoration effect,reduce the incidence of blood sputum and tracheal mucosal injury,shorten the stay time in ICU and mechanical ventilation time,and improve the prognosis of patients.
