摘要
目的分析生物反馈电刺激配合盆底肌训练治疗产后压力性尿失禁(PSUI)的效果。方法选择2018年7月至2020年2月青岛市妇女儿童医院收治的PSUI患者65例为研究对象,按随机数字表法将患者分为对照组(32例)和研究组(33例)进行治疗,对照组采用单纯盆底肌训练,研究组在对照组的基础上采用生物反馈电刺激治疗。比较两组患者的72h排尿相关指标、尿失禁生活质量量表(I-QOL)得分、性功能评估结果、尿动力学指标,并进行统计分析。结果治疗后,研究组总排尿量、漏尿次数、排尿次数均显著低于对照组(t值分别为2.180、10.374、5.968,P<0.05);对照组和研究组治疗后总排尿量、漏尿次数、排尿次数均显著低于治疗前(t=2.029~35.204,P<0.05)。治疗后及治疗前后差值,研究组I-QOL得分均显著高于对照组(t值分别为5.427、17.909,P<0.05);对照组和研究组治疗后I-QOL得分均显著高于治疗前(t值分别为4.102、10.016,P<0.05)。治疗后和治疗前后差值,研究组FSFI各维度得分均显著高于对照组(t=2.054~14.505,P<0.05);对照组和研究组治疗后FSFI各维度得分均显著高于治疗前(t=2.047~14.017,P<0.05)。治疗后,研究组尿道最大测量压、尿道最大闭合压、腹压漏尿点压均显著高于对照组(t值分别为5.672、2.565、4.156,P<0.05);对照组和研究组治疗后尿道最大测量压、尿道最大闭合压、腹压漏尿点压均显著高于治疗前(t=3.357~10.485,P<0.05)。结论生物反馈电刺激配合盆底肌训练,能有效地改善PSUI患者排尿相关指标、性功能及尿动力学指标,提高患者的生活质量。
Objective To analyze the effects of biofeedback electrical stimulation combined with pelvic floor muscle training on patients with postpartum stress urinary incontinence(PSUI).Methods A total of 65 cases with PSUI admitted to our hospital from July 2018 to February 2020 were selected as the research objects.According to the random number table method, the patients were divided into a control group(32 cases) and a study group(33 cases) for treatment.The control group were treated with simple pelvic floor muscle training, and the study group were treated with biofeedback electrical stimulation on the basis of the control group.72 h urination related indicators, urinary incontinence quality of life scale(I-QOL) score, sexual function evaluation results, urodynamic index evaluation results were compared between the two groups.Results After treatment, the total urination volume, the number of leaks, and the number of urinations in the study group were significantly lower than those in the control group(t=2.180,10.374,and 5.968,respectively, P<0.05).The total urination volume, the number of leaks, and the number of urinations were significantly lower than before treatment in two groups(t=2.029-35.204,P<0.05).After treatment and the difference before and after treatment, the I-QOL scores of the study group were significantly higher than those of the control group(t=5.427 and 17.909,respectively, P<0.05).The I-QOL scores of the control group and the study group after treatment were significantly higher than those before the treatment(t=4.102 and 10.016,respectively, P<0.05).After treatment and the difference before and after treatment, the scores of female sexual function inventory(FSFI) dimensions of the study group were significantly higher than those of the control group(t=2.054-14.505,P<0.05).The scores of FSFI dimensions of the control group and the study group after treatment were significantly higher than those before treatment(t=2.047-14.017,P<0.05).After treatment, the maximum measured pressure of the urethra, the maximum urethral closure pressures, and the abdominal leak point pressure in the study group were significantly higher than those of the control group(t=5.672,2.565,and 4.156,respectively, P<0.05).After treatment, in the control group and the study group the maximum measured pressure of the urethra, the maximum urethral closure pressures, and the abdominal leak point pressure were significantly higher than those before treatment(t=3.357-10.485,P<0.05).Conclusion Biofeedback electrical stimulation combined with pelvic floor muscle training can effectively improve the urination related indexes, sexual function and urodynamic indexes of patients with PSUI,and improve the quality of life of patients.
