摘要
目的探讨儿童微指尖再植的临床疗效。方法2018年6月-2019年12月,对8例8指儿童微指尖离断伤进行吻合血管的再植治疗。所有患儿急诊清创后行再植术,术后常规采用拔甲放血治疗,2例出现静脉危象,行小切口放血治疗重建静脉回流。结果8例8指儿童微指尖再植均全部成活,6例常规采用甲床放血治疗,2例采用小切口放血治疗,1例出现皮缘坏死,经换药后愈合。治疗时间5~7 d,平均6.5 d。随访3~12个月,再植指体均发育良好,两点辨别觉5~7 mm,再植指功能恢复良好。结论儿童微指尖离断比较特殊,恰当的手术方式可提高成活率,从而获得良好的治疗效果。
Objective To explore the clinical effect of fingertip replantation in children.Methods From June 2018 to December 2019.In our department,anastomotic vascular replantation was performed for children in 8 fingers of 8 patients with fingertip amputation.All patients underwent replantation surgery after emergency debridement,fingernails were pulled out and bloodletting therapy was performed.Two of them developed venous crisis.We performed a small incision and let out blood to restore venous return.Results All 8 fingertips survived completely after replantation,and six had routine nail bed bloodletting treatment.Two children were treated by small incision bloodletting,one child had dermal border necrosis,which healed after dressing change.The duration of treatment was 5 to 7 d,with an average of 6.5 d,Then we followed up these cases for 6 to 12 months.All the replanted fingers developed well.Two point discriminance was 5 to 7 mm.The function of the replanted finger recovered well.Conclusion Fingertip amputation is special in children,and appropriate surgical methods can improve the survival rate and obtain good therapeutic effect.
