一种用于验证胸腔LDCT-PCNB扫描参数模型的设计与应用

摘要 目的在诸多涉及胸腔低剂量CT引导下经皮穿刺活检(Low-Dose CT-Guided Percutaneous Core Needle Biopsy,LDCT-PCNB)扫描参数的研究里,其设计多为经验法或者穷举法,考虑到不同机型硬件差异,这些研究的通用性及伦理有待商榷。为此设计一款专用于验证胸部LDCT-PCNB扫描参数的体模。方法根据胸腔天然对比以及CT线性衰减原理,通过SKUP(SketchUp草图大师)设计模型,并基于熔融沉积制造(Fused Deposition Modeling,FDM)的3D打印机来制作实体,带入CT进行参数对比试验,对不同低剂量参数结果进行t检验,选择数值较好LDCT参数带入实际扫描,并使用t检验与普通扫描对比。结果经过体模对比试验,两种LDCT参数结果差异较大(P>0.01),而在实际穿刺中,体模实验选择的参数比普通扫描其ED降低了约25倍,且两者质量剂量比差异较大(P=0.01)。结论该体模可一定程度模拟胸腔穿刺的几何关系及相关参数,并择优选择质量剂量比较高的参数用于实际使用,有效减少了重复实验,提高辐射使用效率,并避免伦理问题。 Objective In many studies on scanning parameters of low-dose CT-guided percutaneous core needle biopsy(LDCT-PCNB),the design was mostly empirical method or exhaustive method.considering the difference of hardware of different models,the generality and ethics of these studies were to be discussed.Therefore,a phantom was designed to verify the scanning parameters of LDCT-PCNB.Methods According to the principle of chest natural contrast and CT linear attenuation,a model was designed by SKUP,and the 3D printer based on fused deposition modeling(FDM)was used to make the entity,which was brought into CT for parameter contrast test.The results of different low-dose parameters were tested by t-test,and the low-dose parameters with better values were selected to be brought into the actual scanning,and compared with the ordinary scanning by t-test.Results Through the phantom comparison test,there were significant differences between the two LDCT parameters(P>0.01).In the actual puncture,the ED parameters selected in the phantom experiment were about 25 times lower than those in the conventional scanning,and the difference of the mass dose ratio between the two was large(P=0.01).Conclusion The phantom can simulate the geometric relationship and related parameters of thoracic puncture to a certain extent,and select parameters with high quality and dose for practical use,which can effectively reduce repeated experiments,improve the efficiency of radiation use,and avoid ethical problems.
