摘要
目的对正五聚蛋白-3(PTX3)、中性粒细胞(N)与淋巴细胞(L)比值(NLR)与系统性红斑狼疮(SLE)患者疾病活动度及自身抗体的相关性进行分析。方法选取2019年5月至2021年5月在该院首次确诊的90例SLE患者作为研究对象,将SLE疾病活动度指数(SLEDAI)≤9分的56例患者纳入低SLEDAI组,SLEDAI>9分的34例患者纳入高SLEDAI组。另选取同期90例健康体检者作为对照组。比较3组PTX3、N、L及NLR水平。采用受试者工作特征曲线分析PTX3、NLR对SLE患者疾病活动度的诊断价值。对抗核小体抗体(AnuA)、抗双链DNA抗体(抗dsDNA抗体)阳性和阴性的SLE患者PTX3、NLR水平进行比较。对SLE患者PTX3、NLR水平与SLEDAI、AnuA、抗dsDNA抗体的相关性进行分析。结果高SLEDAI组PTX3、N及NLR水平均明显高于低SLEDAI组和对照组,L水平明显低于低SLEDAI组和对照组,差异均有统计学意义(P<0.05);低SLEDAI组PTX3、N及NLR水平均明显高于对照组,L水平明显低于对照组,差异均有统计学意义(P<0.05)。PTX3和NLR对SLE患者疾病活动度诊断的AUC分别为0.859(95%CI:0.768~0.950)、0.807(95%CI:0.710~0.904)。AnuA、抗dsDNA抗体阳性SLE患者PTX3、NLR水平均明显高于AnuA、抗dsDNA抗体阴性患者,差异均有统计学意义(P<0.05)。SLE患者PTX3、NLR水平与疾病活动度、AnuA、抗dsDNA抗体均呈明显正相关(P<0.05)。结论SLE患者PTX3、NLR水平异常升高,与疾病活动度、AnuA、抗dsDNA抗体有明显相关性,可作为判断SLE患者病情的有效标志物。
Objective To analyze the correlation between pentraxin-3(PTX3),neutrophil(N)to lymphocyte(L)ratio(NLR)and the disease activity,autoantibodies of patients with systemic lupus erythematosus(SLE).Methods A total of 90 SLE patients diagnosed for the first time in the hospital from May 2019 to May 2021 were selected as the research subjects,56 patients with SLE disease activity index(SLEDAI)≤9 points were included in the low SLEDAI group,and 34 patients with SLEDAI>9 points were included in the high SLEDAI group.In addition,90 healthy subjects underwent physical examination in the hospital during the same period were selected as the control group.The levels of PTX3,N,L and NLR were compared among the three groups.The receiver operating characteristic curve was used to analyze the value of PTX3 and NLR in the diagnosis of disease activity in SLE patients.The levels of PTX3 and NLR in SLE patients with positive and negative anti-nucleosome antibody(AnuA)and anti-double stranded DNA antibody(anti-dsDNA antibody)were compared.The correlation of PTX3 and NLR levels with SLEDAI,AnuA and anti-dsDNA antibodies in SLE patients was analyzed.Results The levels of PTX3,N and NLR in the high SLEDAI group were significantly higher than those in the low SLEDAI group and the control group,and the L level was significantly lower than that in the low SLEDAI group and the control group,the differences were statistically significant(P<0.05).The levels of PTX3,N and NLR in the low SLEDAI group were significantly higher than those in the control group,and the L level was significantly lower than that in the control group,the differences were statistically significant(P<0.05).The AUC of PTX3 and NLR for the diagnosis of disease activity in SLE patients were 0.859(95%CI:0.768-0.950)and 0.807(95%CI:0.710-0.904)respectively.The levels of PTX3 and NLR in AnuA and anti-dsDNA antibody positive SLE patients were significantly higher than those in AnuA and anti-dsDNA antibody negative patients,and the differences were statistically significant(P<0.05).The levels of PTX3 and NLR in SLE patients were significantly positively correlated with disease activity,AnuA and anti-dsDNA antibody(P<0.05).Conclusion The levels of PTX3 and NLR in SLE patients are abnormally increased,and they are significantly correlated with disease activity,AnuA and anti-dsDNA antibodies,which could be used as effective markers for the diagnosis of SLE patients.
