摘要
目的探讨注射用烟酸联合西洛他唑治疗动脉硬化性闭塞症(ASO)的效果及对间歇性跛行距离、踝肱指数(ABI)、血脂水平的影响。方法选取2019年1月—2020年12月我院收治的102例下肢ASO患者,根据治疗方法的不同均分为观察组和对照组,所有患者接受基础治疗和口服西洛他唑治疗,对照组在上述治疗基础上联合注射用烟酸治疗,2组均连续治疗8周。观察2组临床疗效,治疗前后右侧胫后动脉和足背动脉内径(d)、收缩期峰值速度(Vs)、间歇性跛行距离、ABI、脂联素、超敏C反应蛋白(hs-CRP)、高密度脂蛋白(HDL)、低密度脂蛋白(LDL)、总胆固醇(TC)、三酰甘油(TG)水平,以及不良反应发生情况。结果观察组治疗总有效率为88.24%高于对照组的66.67%(P<0.01)。2组治疗8周后右侧胫后动脉、足背动脉d及间歇性跛行距离、ABI、脂联素、HDL高于治疗前,右侧胫后动脉、足背动脉Vs及hs-CRP、LDL、TC、TG低于治疗前(P<0.05,P<0.01);观察组治疗8周后右侧胫后动脉、足背动脉d及间歇性跛行距离、ABI、脂联素、HDL高于对照组,右侧胫后动脉、足背动脉Vs及hs-CRP、LDL、TC、TG低于对照组(P<0.05)。2组不良反应发生率比较差异无统计学意义(P>0.05)。结论注射用烟酸联合西洛他唑治疗ASO效果显著,可有效延长患者间歇性跛行距离,提升ABI,改善血流动力学指标、炎性因子,降低血脂,并且不会增加不良反应的发生,安全性较高。
Objective To investigate clinical efficacy of Nicotinic Acid for injection combined with Cilostazol in treatment of patients with arteriosclerosis obliterans(ASO)and its effects on levels of intermittent claudication distance(ICD),ankle brachial index(ABI)and blood lipid.Methods A total of 102 patients with lower extremity ASO admitted between January 2019 and December 2020 were divided into observation group and control group according to different therapeutic methods.All patients received basic treatment and oral Cilostazol treatment.Control group was treated with Nicotinic Acid for injection on the basis of the above treatments.Both groups were treated for 8 consecutive weeks.Clinical efficacy,levels of diameter of right posterior tibial artery and inner diameter of dorsalis pedis artery(d),peak systolic velocity(Vs),ICD,ABI,adiponectin,high-sensitivity C-reactive protein(hs-CRP),high-density lipoprotein(HDL),low-density lipoprotein(LDL),total cholesterol(TC),triglyceride(TG)before and after treatment,and incidence rate of adverse reactions were compared between two groups.Results The total effective rate in the observation group(88.24%)was significantly higher than 66.67%in control group(P<0.01).After treatment for 8 weeks,levels of right posterior tibial artery,dorsalis pedis artery(d),ICD,ABI,adiponectin and HDL were significantly higher,while levels of right posterior tibial artery,dorsalis pedis artery Vs,hs-CRP,LDL,TC and TG were significantly lower than those before treatment in two groups(P<0.05,P<0.01).After treatment for 8 weeks,in observation group,levels of right posterior tibial artery,dorsalis pedis artery d and ICD,ABI,adiponectin and HDL were significantly higher,while levels of right posterior tibial artery,dorsalis pedis artery Vs,hs-CRP,LDL,TC and TG were significantly lower than those in control group(P<0.05).There was no significant difference in incidence rate of adverse reactions between two groups(P>0.05).Conclusion Nicotinic Acid for injection combined with Cilostazol is effective in treatment of ASO.It may effectively prolong ICD,improve ABI,hemodynamic indexes and inflammatory factors and reduce blood lipid without increasing incidence rate of adverse reactions with good safety.
